**37 hours per week, 1.0 FTE, £33,568 per annum**

Sheffield Futures is a dynamic and vibrant independent charity, which supports young people and adults to achieve their full potential in learning, employment and life to achieve a better future. The majority of our work is with young people aged 13 - 25 who we aim to support and equip for a positive transition into adulthood.

We are currently recruiting for the role of Social Prescribing Coordinator to coordinate the delivery of Door 43’s Social Prescribing Service. In conjunction with the Counselling and Youth & Wellbeing Coordinators, the role will deliver in the context of the Youth Information and Counselling Service (YIACS) model, including planning, communication, performance management, risk analysis, monitoring and evaluation. The Coordinator will support the development of pathways and processes which enable young people to access a variety of support with specific focus on emotional health and well-being.

The successful applicant will be responsible for the line management of a skilled team of Social Prescribing Link Workers and Health and Wellbeing Practitioners, who work across the city in multiple venues. This role works in a hybrid way from Star house in Sheffield City Centre and remotely from home. All Coordinators are part of a duty rota at Sheffield Futures and will at times be required to work evenings during the week.

This role requires an enhanced DBS check.
