The PeakEdge PCN is a network covering over 58,000 patients in Sheffield. We are agrowing team of pharmacists and Pharmacy Technicians who are innovative andcommitted to delivering high quality care for our patients. We are seeking apharmacy technician to complement the team and to play a key role in deliveringthe PCN agenda.

Main duties of the job

Support the delivery of clinicalpharmacy services such as reviewing clinic letters and hospital dischargesummaries for changes to medication, either new medication added, or medicationdiscontinued. It is also expected that any monitoring requirements arising froma secondary care letter will be noted. Several medications have shared-careprotocols outlining monitoring requirements, any new shared-care arrangementshould be agreed by a GP familiar with the shared care protocol. Read codes areoften used to add a patient to a particular monitoring register.

Increase the quality and safetyof prescribing through medicine safety initiatives and project work.

Deal with patient and healthcareprofessional medication-related queries

Action MHRA and CAS Alerts.

Job responsibilities

Undertake patient facing andpatient supporting roles to ensure effective medicines use, through shareddecision-making conversations with patients.

Carry out medicinesoptimisation tasks including effective medicine administration ( checkinginhaler technique), supporting medication reviews and medicines reconciliation.Where required, utilise consultation skills to work in partnership withpatients to ensure they use their medicines effectively.

As determined by the PCN,support medication reviews and medicines reconciliation for new care homepatients and synchronising medicines for patient transfers between caresettings, linking with local community pharmacies

Support the clinical pharmacistto deliver structured medication reviews (SMRs) , organise necessary monitoringtests prior to SMR.

Provide expertise to addressboth the public health and social care needs of patients, including lifestyleadvice, service information, and help in tackling local health inequalities.

Manage shared care protocols andliaise with clinical pharmacists for more complex patients.

Support initiatives forantimicrobial stewardship to reduce inappropriate antibiotic prescribinglocally.

Support the PCNmulti-disciplinary team to ensure efficient medicines optimisation processesare being followed.

Implement efficient orderingprocesses and reduce medication wastage.

Provide training and support onthe legal, safe, and secure handling of medicines, including the implementationof the Electronic Prescription Service (EPS).

Promotion of Electronic RepeatDispensing (eRD) and online ordering

Develop relationships with otherpharmacy professionals and members of the multidisciplinary team to supportintegration across health and social care including primary care, communitypharmacy, secondary care, and mental health.

Support practice reception teamsin streaming general prescription requests, to allow GPs and clinicalpharmacists to review the more clinically complex requests.

Support the implementation ofnational prescribing policies and guidance within GP practices, care homes andother primary care settings.

Support the PCN to deliver onQIPP agenda, QOF and locally commissioned enhanced services.

Support the PCN in reviewing anddeveloping practice policies for CQC requirements.

Person Specification

Experience

Essential

Good clinical pharmacy knowledge including terminology. An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for the improvement of prescribing. Knowledge and understanding of pharmacy law and ethics and current legislation. Has attention to detail, able to work accurately, identifying errors quickly and easily. Able to effectively manage allocated resources. Has a planned and organised approach with an ability to prioritise their own workload to meet strict deadlines. Able to think analytically; anticipating obstacles and thinking ahead; using analytical techniques to draw logical solutions to problems. Excellent communication skills, verbal and written, with the ability to adjust communication style and content to suit the audience. Excellent verbal and written communication skills with team members, patients, carers, and other healthcare professionals. Whilst recognising peoples needs for alternative methods of communication. An excellent understanding of data protection and confidentiality issues. Able to travel across the PCN.

Desirable

Experience of working as a qualified, registered pharmacy technician in primary care, community or hospital pharmacy

Essential

Qualifications & Experience: Professional registration with GPhC BTEC/NVQ level 3 or equivalent in pharmaceutical sciences Evidence of continued professional development (CPD)

Desirable

Demonstrate ability to influence and persuade partners and stakeholders of the respective merits of different options, innovations, new opportunities and challenges.
