Health and Wellbeing Coach for Young People

Wisbech Primary Care Network (PCN) is recruiting a Health and Wellbeing Coach for Young People to support patient care in the development of the Primary Care Network. The PCN has a population of approximately 50,000 patients and comprises of 4 Practices North Brink Practice, Trinity Surgery, Clarkson Surgery and Parson Drove Surgery. The post holder will work alongside clinicians and other staff across the Wisbech Network as part of a multi-disciplinary team. Our Support and Early Engagement Network [SEEN] is led by a registered nurse with health coach training, in addition the supporting team consists of social prescriber/s and health and wellbeing coaches to ensure personalised care for those accessing the service.
Experience in both a primary care setting and/or childrens wellbeing, would be an advantage but not essential for this supporting role. Initiated by the Wisbech Primary Care Network, SEEN is focused on ensuring that children and their families have access to wellbeing support. With an aim to reduce rejected referrals, ensuring earlier intervention and providing help to navigate the local and national services available in the Wisbech area. Children, adolescents, and their families will have quicker access to high-quality, age-appropriate wellbeing support if the assessment is thorough, timely and holistic.
Additionally, great communication skills are essential to allow for collaborative working. Under supervision of the Lead Health & Wellbeing Coach, frequent communication with schools, secondary care services, safeguarding teams, GP practices, community services, the County Council-targeted support teams, local charitable organisations, local paediatricians, lead clinicians and the ICB. Additionally, working cohesively within a team, under supervision to provide support to promote best practice and wellness for all. SEEN is a growing team that connects services together.
This primary care initiative reduces the burden on GPs, and allows continuity of care. Offering support around services, bridging gaps in care and offers services such as coaching and social prescribing to young people and their families. The job role remains fluid, but is underpinned by SEENs Aims and Objectives: SEEN's mission is to promote wellness in Wisbech by offering biological, psychological, and social assistance to young people and, if feasible, their families via a network of teams that include primary and secondary care services with the goal to reduce mental health conditions. The role will be varied and include offering holistic wellbeing assessment of young people aged 0 to 19 through our single point of access clinic based at a Wisbech GP practice. Gathering information to understand how best the young person and family can access support; this role will entail referring internally to our SEEN team as well as externally to specialist services such as secondary care/ community services. Working with a multidisciplinary team to advocate for young people is paramount, gathering information and assessing the most appropriate route for support and/or intervention. The assessment will be based on the biopsychosocial model, exploring holistically the factors that influence wellbeing, such as; physical, emotional, intellectual, vocational, economic, social, spiritual, and environmental factors.
In addition, the post holder will provide coaching support to young people aged 12 to 19 working in partnership with clinical and non-clinical professionals, social care and voluntary sector colleagues to ensure the best possible outcomes for the young person. Also offering support to our team to facilitate well-being drop-ins and peer support groups. Main duties of the job Take referrals from the SEEN single point of access when it has been identified that patients meet the criteria for health and wellbeing coaches Work in partnership with clinical and non-clinical colleagues, to explore all contributing factors to wellbeing. Support patients to discuss goals, progress and difficulties.
