Social Care Assessor

Due to our unique offering, we continue to expand the business and wish to appointa number of**Assessors**to join our team of health and social care experts. We require people who are**qualified mental health or learning disability nurses, social workers and occupational therapists**with a strong background inservice user needs assessment and care formulation.
Working within a geographical region and at times covering other areas, travel will be required as part of the role and along with being a car driver, an organised and flexible approach with ability to stay overnight as required will be critical.
**Main Responsibilities**
As Assessor you will lead on clinical assessments ensuring that we are in a position to deliver individualised support packages. You will then support the teams to transition the person to Gray Healthcare and commencement of their care package. You willwork closely with the business development and operational teams across an agreed geographical area.
As a passionate professional you will have in-depth experience of assessing and working with people with a variety of complex needs around their mental health, learning disability, ABI, personality disorder, autism and forensic backgrounds. You will havean understanding of continuing healthcare or transition of individuals moving from hospital/residential settings into the community and a collaborative, solutions-focussed approach to any issues associated with this.You will use your applied knowledge of theMental Health Act, CTO’s and the Mental Capacity Act, Deprivation of Liberty Safeguards and Best Interest Decision making to inform the direction of packages in your remit.
An expert in assessment and management of risk you will be an innovative thinker and natural communicator with strong report writing skills and computer literacyand an ability to work under pressure.
