**Reports into**: Case Management Assistant Lead

**Role Objective**: To provide administrative and technical support to a group of Rehabilitation Case Managers, enabling operations to run smoothly and efficiently, assisting with ensuring that clients are discharged with the highest potential return to work outcomes.

The role of Case Management Assistant (CMA) within Innovate Healthcare is responsible for the provision of administrative and technical case support to Case Managers in the Rehabilitation Case Management team.

Reporting into the Case Management Assistant Lead, the Case Management Assistant will build strong working relationships with the case managers and the Operations Manager. The role will be responsible for helping to ensure that our clients are able to recover swiftly from injury and return to work within the best possible timeframe. The role will encompass a great deal of team work, but also requires the need to work autonomously in order to ensure that Innovate Healthcare continue with our record of delivering the highest quality customer service.

The Case Management Assistant role sits within a team of other CMAs and, whilst has specific responsibility to individual Case Managers as assigned, will also be providing ad hoc cover and support to other Team members as required. Part of a growing team, the role has ample opportunity to contribute to the development and continuous improvement of internal processes and procedures and to shape the future of Case Management within the organisation.
- Writing and sending referrals for treatment services
- Pre-populating templates with detailed and sensitive information
- Booking in assessments
- Requesting relevant medical notes post-assessment
- Transferring of information into case notes on a case management system
- Chasing various parties for authorisation
- Proof reading of reports for grammar error
- Distributing reports to stakeholders
- Invoice generation from a management system
- Updating Management Information fields on a management system
- Closing and finalising of files
- Distributing consent forms and organising reminders for their return
- Regular progress calls with injured parties that case coordinators are monitoring whilst treatment is ongoing
- Booking venues, accommodation and processing travel requests
- Liaising with colleagues in other departments for updates on treatment and reports
- Contacting GP’s for consent to make diagnostic referrals
- Setting for tasks for clinicians to review received information and to make follow-up calls
- Responding to tasks set by the clinical consultant
- To work on and towards the Innovate Mission and Vision and commit to promoting the organisation’s culture and values

- Highly developed administrative skills, with a passion for attention to detail

- A good telephone manner and high-class organisational skills
- Ability to self-motivate and structure your own workload
- Flexible and adaptable, used to regularly having to realign priorities
- Skilled at communicating in different formats and building rapport across a wide spectrum of stakeholders
- Enjoys the opportunity to work a mix of independently and as part of a wider team
- Ability to work under pressure and to strict deadlines
- Good IT skills, with a knowledge of MS Word, Excel and Outlook and ideally Microsoft Dynamics.
