Working as a member of the Hospice Day Care and Outreach Team, supporting a caseload and carrying out assigned tasks associated with the care and support service users, their family carers and staff who support them. Offering support to the End of Life Care Admiral Nurse and Hospice Outreach Manager with community projects. To increase the hospice links with our local community and minority groups and assisting in reduce barriers to hospice care.

The post holder will be able to work independently and will receive support and supervision from the Hospice End of Life Care Admiral Nurse and Hospice Outreach Manager.

Additionally, will assist in the planning and delivery of training for Day Care Services for all salaried and volunteer members of the Hospice team and with any Carers Education Programmes. Participate in structuring, developing and delivering the Wellbeing program for attendees of Day Therapy services, liaising internally and externally with other health and social care professionals.

1. Assisting the Hospice Day Therapy and Outreach Team in providing care and support to people with dementia or other life limiting conditions, their family carers and staff who support them.

2. Maintaining effective communication both face to face, virtual and written.

3. Have effective triaging skills and know when a person needs to be referred to others.

4. Liaison with family carers and staff who are supporting our day care service users to ensure the feeling of wellbeing and comfort.

5. Support the Day Care Teams with life story work, reminiscence therapy and other aspects of Namaste Care such as ‘M’ technique.

6. Support the Hospice Day Therapy and Outreach Teams with discussions and activities around Advance Care Planning, Carers Contingency plans etc.

7. Co-operation with the promotion of good staff morale, supporting and assisting all grades and voluntary staff, as appropriate.

8. Establishing good liaison with family carers, staff who support them and to all visitors to the Hospice.

9. Actively being involved with the Hospice Outreach Manager to make contact with hard to reach groups who historically may not have accessed hospice services.

10. Being an ambassador for the hospice at public events to promote our services and explore perceived barriers to accessing hospice services.

11. Ensuring that any complaints are met with a tactful and sympathetic response and that they are reported in accordance with Hospice policy.

12. Reporting accidents involving staff, patients and visitors to the Hospice/environment and submitting written reports on request.

13. Participating in relevant weekly meetings with the Hospice Day Therapy Teams.

14. To plan and help deliver in-service dementia awareness training for all salaried and volunteer members of staff in conjunction with the Hospice End of Life Care Admiral Nurse.

15. Provide well-being support telephone calls.

16. To help deliver the Carers Education Programme alongside the Hospice End of Life Care Admiral Nurse/ Hospice Dementia Advisor.

17. To act as a resource for staff and family carers for end of life advice and support.

18. To maintain evidence based practice by attending training/ annual update sessions and to be aware of current Government legislation.

19. Being aware of the importance of health and safety of patients and staff and acting accordingly.

20. Observing Hospice fire precautions and drill procedures.

21. Participating in the ordering of stock and new supplies, as required, with due regard to economy.

22. Being aware of the importance of efficiency and cost effectiveness within the Hospice and acting accordingly. Support the Hospice with Income Generation where appropriate.

23. Treating all Hospice matters, including information regarding patients, their families, their illness and treatment, with utmost confidentiality.

24. Be prepared to work on the inpatient unit if required

25. Be flexible to working hours, this may involve working some weekends and bank holidays

26. Any other tasks which may from time to time be requested.
