Equal Lives is a user-led disability rights organisation, working across Norfolk and Suffolk. We’re looking for someone to join our team of Living Well Workers within the Norwich Social Prescribing Project.

**Job Purpose**

The role of the Living Well Worker is to support the provision, coordination and development of Social Prescribing, supporting individuals and families to access resource within their locality from community, voluntary and public-sector organisations.

The Living Well Worker will contribute to the wider early help and prevention approach by supporting individuals; the consequential impact being the development of strong, resilient and sustainable communities.

The role requires close working with health and social care providers to maximise the individual’s potential to reduce or delay more formal packages of care and to reduce demand in primary care and other statutory services.

**Job Summary**:
As social prescribing will be continuously evolving, you will also be feeding back what works well in order to develop models of good practice. This will be done via regular Living Well Team meetings and through quarterly supervisions. Where possible, you will work closely with GP practices and clinicians as well as multi-disciplinary teams to achieve positive, sustainable outcomes for your clients.

**Responsibilities**:

- To receive referrals from Integrated Care Co-ordinators and other referral pathways
- To work flexibly from various locations including GP practices, provider offices and in some instances from home
- To conduct initial client interviews and detailed diagnostic interviews which proactively and holistically identify the needs of individuals and/or their families
- To deliver time limited support through a person-centred approach, enabling individuals to identify suitable solutions and make informed choices
- Where appropriate, to undertake casework and negotiation on a client’s behalf
- To make referrals to voluntary sector services, community groups and, where relevant, public sector services
- To work as part of the Living Well Team, giving and receiving support in a cooperative and collaborative way
- To receive support and supervision according to our policy and procedures and to attend Living Well Team meetings
- To participate in the appraisal scheme and undertake training appropriate to the development of your job role and in line with the employing organisations’ statutory obligations
- To keep up to date with knowledge of the voluntary sector and community groups in your locality
- To develop and maintain a working relationship with relevant local organisations in the statutory, private and voluntary sectors, plus relevant staff from primary care and broader health and social care services
- To provide an outreach service in health and community settings to maximise access by the target client group
- To manage and maintain a caseload
- To complete accurate written records and maintain case files in accordance with the requirements of the organisation’s quality standards
- To record statistical information, accurately and regularly in accordance with project requirements and procedures
- To keep up to date with relevant legislation and case law by attending training and using reference materials/journals
- To report on any gaps in community resources that become apparent when carrying out social prescribing
- To work collaboratively with health and social care colleagues

To undertake any other duties commensurate with the post as agreed with the line manager.
