If so, we would really like **you** to be part of one of our Health & Adult Services (HAS) Occupational Therapy teams to support in a focused OTA/ Trusted Assessor role. You will support the team/service to achieve their commitment to reducing assessment wait times for people in our community which is linked to one of the HAS key improvement priorities to ‘Prevent, Reduce, Delay’. This will involve holding your own caseload in a supported way and assessing (under the Care Act 2014 and Housing Construction and Regeneration Act 2006 principles) people with a variety of disabilities, their Carers and families within their own home to look at creative/home adaptation solutions to support their needs long term and overall improve their quality of life and reduce the need for ongoing care and support.

We are looking for people with creative thinking, effective problem solving and observational skills to join and complement our Occupational Therapy Teams.

Whilst located within one of the Occupational Therapy Teams, you will also be a key link between our Reablement and Community Social Care Teams to ensure people are supported in the right place at the right time with the right intervention/s, demonstrating how adapting a person’s home environment with the right design can have such an impact on a person’s life.

You will work innovatively to offer information, advice, and guidance and/or signpost people to local services, working with our partner agencies. You would be joining us at a pivotal time for North Yorkshire Council's unitary development as we move towards our 2025 plan and key improvement priorities. You will be based in one of the above locality teams, managed/ supervised by the locality Occupational Therapy Team Manager with caseload support /oversight of experienced Occupational Therapists and Senior OT’s. You will be part of a supportive and creative team and encouraged to ‘think outside of the box’.

**This is an entry level role into the Occupational Therapy profession which we anticipate will appeal to those wanting to develop skills which will support them to progress a future career in Occupational Therapy or alternatively for experienced Health and Social Care practitioners who are interested in new opportunities and developing their skills in this area.**

**Job specifics**:

- You will be involved in assessing people with low to moderate level equipment, minor and major adaptation needs and be a key link to our new in-house Disabled Facilities Grant (DFG) Housing Teams
- To analyse a persons’ activity and identify a graded/ person centred approach to promote their independence and achieve the things that matter to them.
- To promote people's independence and 'home first' approach.
- To provide effective information, advice, guidance and signposting
- To provide Occupational Therapy Conversation Record style assessments within the locality OT team and to link where required with all relevant teams
- To complete assessments, considering mental capacity, best interests and relevant risk assessment where required and provide effective and timely solutions/ support plans alongside people/carers with a preventative focus including Technology Enabled Care (TEC).
- To work collaboratively with colleagues, Reablement and Recovery delivery arm to ensure that an adult with care and support needs and/or their carers outcomes are achieved.
- To pro-actively work in partnership with Social Workers, Social Care Coordinators, Independence Coordinators/ Reablement colleagues, Sensory Team colleagues and necessary links with Mental Health teams.
