We are looking for a highly motivated Therapy Assistant to join the Surgical Emergency Unit team.

You will work under the supervision of the therapists and registered nurses, carry out rehabilitation plans and help facilitate discharges, for patients within the Surgical Emergency Unit. Working alongside the discharge coordinator, you will participate in early identification of patient needs, arrange follow on care services and refer patients appropriately via intranet and in accordance with trust policy.

You will need to have excellent communication and organisation skills, be a team player and be able to use your own initiative. You will be supported by qualified therapists and work within a competency framework. You will manage your own workload and participate in team and service development.It is essential to have completed the Foundation Degree in Health & Social Care - or equivalent(if you do not hold either of these qualifications, you will not be shortlisted).

Detailed job description and main responsibilities
- To assist the nursing and therapies team in the management of both simple and complex discharge planning.
- To work closely with the discharge co-ordinator and complete tasks on EPR relevant to the Therapies Assistant role.
- Perform, understand and report patient risk assessments.
- Perform additional tasks and clinical skills as directed by the registered staff following completion of relevant competencies.
- To encourage and support patients through their hospital journeys - focussing on the early rehabilitation, mobilising of patients and discharge planning.
