An exciting opportunity has arisen to support the trust through the winter to improve the discharge process and support flow through the organisation. This role will support the existing team of discharge co-ordinators to plan and manage timely and appropriate transfers of care (discharge) for patients from the hospital; working to support the Trust goal to increase the number of discharges/transfers taking place before 10 am. The role will involve focusing on patients that no longer meet the criteria to reside in an acute hospital bed and working towards a same day discharge. The post holder will work as part of the ward team (Clementine A) to focus on early identification of potential discharge planning issues or complex needs, by communicating with patients and families/carers on admission. The role will link in with multidisciplinary team and system partners to ensure smooth and safe discharges. This will involve working with system partners to ensure plans are in place and well communicated to both patients and their families/carers. This role will be supported by the wards, Red2Green team and Geriatrics Matrons.

Co-ordinate all discharges within specified wards, with particular focus on patients that no longer meet the criteria to reside in an acute bed.

Develop effective links and relationships with community health and social care teams and relevant professional partners, acting as their principal point of contact, to ensure that all discharge arrangements are agreed and information flows effectively, to provide a seamless and safe service for patients and their families.

Liaise with Local Authorities, nursing homes and other relevant units regarding patient placements. Assist ward staff to implement best practice in planning safe and timely patient discharges, optimising hospital bed utilisation and reducing length of stay, whilst ensuring the best outcomes for patients. Assess, monitor and report on patient length of stay and partake in weekly length of stay reviews in collaboration with the Red2Green team. Provide advice and support to the wards and their multi-professional teams in managing safe and effective patient discharge as an integral part of care management across the Trust. Monitor and maintain patients EDD, criteria to reside and discharge plans on the patient flow whiteboards, escalating delays via Red2Green reporting process.

Our services include all the major specialties of large acute hospitals, operating from two main sites - King George Hospital in Goodmayes and Queen’s Hospital in Romford. We also provide outpatient services at Brentwood Community Hospital, Barking Hospital, Loxford Polyclinic and Harold Wood Polyclinic.
