**JOB TITLE**: Ward Host GRADE: Band 4 HOURS OF DUTY: 08.30am-4.30pm Monday - Friday. REPORTS TO: Ward Manager/Band 6s KEY RELATIONSHIPS: Patients, carers, Internal and External care providers and volunteers RESPONSIBLE FOR: Delivering high quality specialist patient care on the Inpatient unit and occasional support for Referrals HUB/Community team Administrative Administrative duties associated with Admissions: 1. Creating admission packs, bed checklist, wrist bands etc. 2.

Preparation of documentation and making up notes. 3. Welcome of patients and relatives. 4.

Liaison with referrals team & booking of ambulance service, hospital wards etc. 5. Printing of address labels/ name tags & bed label. Administrative duties associated with discharges and death; 1.

Communication to appropriate people (District Nurse, GP Surgery, Social Workers etc.) 2. Completion of sections of discharge forms. 3. Gathering of documents for patients to take home checking documentation is signed.

4. Prepare tissue donation guidance and info. 5. Add discharge summary to UCP.

10. Liaison with funeral directors. 11. Preparation of cremation forms Part 1.

12. Updating ICARE (patient information records) with coroners referrals. 13. Type up discharge death letters, update ICARE and UCP records.

14. Dismantle notes & file. 15. Submitting CQC notification forms such as Death and Serious Incidents.

Patient Related Duties 1. Ensure daily activity sheet is updated update bed plan & tissue donation list. 2. Update bed plan daily & doctors board.

3. Attend weekly MDT on Wednesday and record information to iCare. 4. Management of newspapers.

5. Coordination of Patient transport with Ambulance Service & Referrals HUB. IPU responsible for booking discharge ambulances/transport home from IPU. 6.

Work proactively to maintain effective communication networks with all departments within the hospice to maintain a seamless delivery of specialist palliative care services. Main Responsibilities 1. Management of patient register. 2.

Prepare files and sets of case notes as required. 3. To maintain effective communication with patients, relatives and other visitors. 4.

To update effective communication with patients, relatives and other visitors. 5. To update the ICARE and be proficient in the workings of UCP, cyberlab to ensure accurate up-to-date information. 6.

To generate reports as required for the inpatient unit. 7. Maintain efficient clerical systems. 8.

To work closely with the Supplies Manager when ordering office supplies. 9. Provision of secretarial services to Head of Quality and Audit working in a clinical and non-clinical environment. 10.

Typing correspondence and minutes, including copy and audio typing, using Microsoft packages such as Word/Excel/Outlook etc and other databases used by the hospice. 11. Undertaking administrative/organisational duties on behalf of Hospice Managers. 12.

Daily checking of PCR results and uploading to iCare. 13. Input of weekly patient IPOS. 14.

Order death certificate books / crem forms from registrar. 17. Preparing & making up of controlled drug logs, DAD log & Covid logs. 18.

Administration support of weekly MDT meeting. Communication 1. To maintain effective communication and liaise with hospitals, clinics, secretaries regarding appointments, notes and doctors letters. 2.

To ensure adequate supplies of stationery, bereavement information packs and other forms as required. 3. Make and receive telephone calls, take accurate messages using hospice procedure and ensure delivery of messages. 4.

Keep notice boards tidy and up to date. 5. Liaise with Pemberton Place and pharmacy regarding collection of patient medication. 6.

To liaise with Finance department regarding, donations, cremation fees and other monies. 7. Once directed to contact nursing bank staff to arrange cover for the ward. 8.

To work closely with the Senior Ward Sister on any developments and initiatives as requested. Education of Self 1. Actively participate in the Saint Francis Hospice Schwartz practice programme and meet mandatory attendance requirements. 2.

To take responsibility for own development which will be monitored as part of the annual appraisal review.
