The Access and Assessment Team (AAT) has an exciting opportunity for a Clinical Specialist Practitioner focusing on the Rough Sleeper Population with Dual-Diagnosis. This is a fixed-term contract with funding for 2 years.

The successful applicant will work within the mental health AAT Team as a specialist dual diagnosis worker to ensure that rough-sleeping clients are seen quickly. The post holder will understand and accommodate the challenges and chaotic nature of the lives of this population and build trust with clients. They will also deliver services and interventions as part of the wider outreach team.
- Provide leadership to the clinical team in the care, treatment and therapy of the rough-sleeper client group.
- Identify, assess and coordinate the long-term support needs of individuals with a dual diagnosis and other associated complex needs with the aim of facilitating improved outcomes.
- Work as part of a wider MDT to provide the specialist outreach support to engage rough sleepers and those at risk with drug/alcohol issues.
- Influence, lead and motivate staff within the clinical environment to deliver high quality care.
- Provide support and advice to clinical practitioners regarding clinical care including implementing best practice based.
- Co-ordinate the multi-agency approach, including sharing information to support engagement.
- Sustain the clinically based training programme aimed at the development of staff within the clinical environment.
- Provide information, education and support for service users and their carers re coping with diagnosis and treatments.
- Develop a rapid access pathway into prescribing, detox, and rehab programmes as appropriate and to manage associated risks.
- Maintain own expert status by developing training and improvement projects.
- Contribute to relevant forums and meetings.
- Take part/facilitate audits.
- Take part in RCA reviews and implement any lesson’s learnt from incidents/complaints.
- Be accountable for accurate mental health clustering of all patients in caseload on admission, discharge and at review meetings.

**Areas of Responsibility**:
Leadership
- Provide advice and leadership to clinical staff in areas related to clinical care including, rough-sleeping, dual-diagnosis, substance misuse, therapy and clinical risk assessment and management
- Provide consultation support and supervision to teams on issues relating to nursing practice.
- Facilitate joint working arrangements between the Rough Sleeper Service and Drug and Alcohol Recovery services to ensure that individuals with a dual diagnosis do not fall through the net of services, by identifying, monitoring and responding to the needs of this group.
- Work alongside the both the clinical care team and relevant teams including governance and research department to utilise information and data to support the implementation and embedding of best practice for rough-sleepers who have a mental illness.
- Develop process and systems to share learning from local and national research and National guidance
- Ensure any learning is embedded and sustained in clinical services

Clinical
- Seek out and receive appropriate level training to keep abreast of changing trends and practices
- To build collaborative partnerships with Drug and Alcohol Recovery services within Norfolk enhancing care pathways for those with a dual diagnosis.
- Adhere to all Trust policies and procedures contributing to policy development via Governance processes, particularly in relation to clinical care
- Demonstrate a sound knowledge of current legislation and policy that underpins mental healthcare delivery including all aspects relating to the clinical area of practice.
- Understand and appreciate the influences that training has on service delivery and offer specialist guidance, advice and support on improving service and developing services.
- Participate in clinical audit, practice research initiatives, quality assurance and clinical governance.
- Lead the development of training within the local environment to deliver high quality clinical care
- Give a specialist support service to clinical areas throughout the Trust.
- To discharge statutory social care with Norfolk County Council for Service Users and Carers on behalf of the organisation (For Norfolk Service Users only)
