A fantastic opportunity has arisen for an efficient, organised, Care Coordinator to join an expanding domiciliary care company in Wiltshire

The role of the Care Coordinator combines both coordinating and rota planning, care planning as well as personal and domestic carer duties with the objective of helping our Service Users to take care of themselves at home.

As a Community Care Worker your role will include the following duties:

- Assisting the service user with bathing, grooming and getting dressed
- Handling household tasks like grocery shopping and laundry
- Preparing and serving meals at the appropriate time
- Administering oral and topical medication under the supervision of medical personnel
- Providing mental and emotional support
- Making recommendations to family members and healthcare personnel on the plan of care
- Organising suitable recreational activities for the patient
- Collaborating with other health care and social care professionals to provide the best possible care

This is a full or part-time permanent position with variable hours. We are an excellent company who encourage their staff to reach their full potential. **Essential**:

- Own a Full U.K. driving licence and own car

**Desirable**:

- Experience working in the care sector
- Qualification: Level 2 or 3 Diploma in Health and Social Care
