The Patient Pathway Co-ordinator (PPC) is responsible for ensuring the safe and efficient management of the patient throughout their pathway, ensuring potential breaches of RTT (referral to treatment) waiting times targets are avoided or escalated to the relevant manager.

The post holder will be responsible for ensuring patients’ next event is booked in a timely manner and that all data records relating to Referral to Treatment (RTT) times for patients are accurate, up to date and reflect national and local policies. The post mainly covers patients in their non-admitted pathway, but is also there to support as required with the admitted pathway, and is vital to support the trust to manage its access targets relating to 18weeks.

The PPC will work across the specialities of General Surgery, Urology, Colorectal and Breast.

To ensure patients are scheduled in accordance with principles and target waiting times as set out in the NHS Constitution and Health and Social Care Bill 2011 (e.g. 18 Week Referral to Treatment,, expediting patient pathways where necessary. Ensure cancellations are rescheduled and recorded in accordance with Trust policy.

Be fully proficient on all patient information systems throughout the Trust in order to extract information and monitor patient’s pathways. (i.e PiMS/PAS/EPR system(s) including Choose and Book) (all soon moving to EPIC).

Be responsible for ensuring that patient records have the correct clock starts and clock stops, by validating and interpreting patient pathways from first outpatient appointment, through diagnostics, treatment planning and treatment. This may involve adjusting / correcting patient referrals where inaccurate entries have been put in by other members of staff. This may also include amending clinic outcomes or waiting list removal reasons to accurately reflect the patient’s pathway.

Be responsible for ensuring patients’ next event is booked in a timely manner through liaising with speciality booking and schedule teams.

Be responsible for ensuring patients’ diagnostic and investigations have been requested and completed and that results are available for consultations.

Meet personal targets for validation volumes and accuracy, as agreed with the Assistant Service Managers.

Liaise with speciality admissions and outpatients booking teams to support continuous improvement in understanding of RTT rules and data quality.

Develop a specialist understanding of patient pathways specific to specialities.

Highlight any systematic data recording errors or performance concerns to the Assistant Service Managers.

Be responsible for liaising with staff from within the trust and referring organisations to collect accurate clock starts, stop and tertiary data sets and to provide stage of pathway information. Ensure that where patients have been referred for inpatient diagnostic/treatment that the clock start date has been clearly identified.

Ensure that Assistant Service Managers are aware in a timely manner of any previously unreported potential breaches of waiting times targets that are caused by validation.

Analyse data and produce ad hoc reports as directed by Assistant Service Managers

Liaise with clinical/diagnostic teams to ensure that all diagnostic appointments are booked within an appropriate timescale and results are available.

Work with performance and information staff to ensure existing and future reports and monitoring tools is of the highest quality and fit for purpose.
