We have a really exciting opportunity to join a great homelessness Charity based in East London as a Rough Sleeper Engagement & Referral Navigator. The aim of the service is to support and enhance the quality of life for all locally connected rough sleepers on the area. The team offer a through care service from initial engagement and harm reduction, then moving into treatment, and then recover.

The role of the Navigator will involve (but is not limited to):

- Navigating London Borough for Tower Hamlets, working with clients who have a higher complexity in substance use with the aim of a referral into the treatment team
- Conducting in-reach and outreach
- Managing a caseload of 15-20 clients
- Attending partner agency meetings relating to rough sleeping & hot-spots
- Signposting to other support services
- Providing targeted harm reduction advice and individuals

Please note, there will be some evening, weekend and early morning working.
