We support and care for vulnerable adults who require assistance in performing their daily task due to an impairment such as Mental Health, Autism, Learning Difficulty, or Care of the Elderly, Etc.. in the community. **Care Coordinator requirements**
- Experience in the care sector for 12 months and above.
- Skills and Knowledge in care are essential and willing to learn new things.
- We are currently in search of a someone who has good communication skills, compassionate, caring, competent, courage, flexible, committed and reliable Care Coordinator for the purpose of care delivery in London and the surrounding Boroughs.

**Care Coordinator Day to Day duties**:

- The applicant will work as part of the team and be reporting to the Care Manager.
- Your role will be promoting person-centred care for the Service Users and assisting/supporting the team in meeting the needs of the Service Users.
- Regular communication with carers and Service Users
- Deliver high quality care and support
- Participate in the on-call rota
- Generating care plans
- Completing Rotas with our Digital system
- Completing audits
- Liaising with the External MDT's involve in SU's care
- Seize opportunities to assist in other areas of the business that match your skillset, such as completing assessments or representing the company at external events.
- Administrative/IT skills and knowledge etc
- Knowledge of legal responsibilities and regulations in health and social care
- Knowledge about person-centred care planning
- Good organisational and communication skills
- Driving is essential due to the nature of the role.
