What you will be doing?:
Reporting to the Team Leader and Service Manager of the new Housing Support Team, you will be responsible for co-ordinating and providing a personalised, trauma informed service to people with a drug and alcohol issue that are experiencing issues within their tenancy linked to their substance misuse. You will provide intensive and consistent support to create long-term sustainable tenancies. You will promote independence, whilst supporting individuals and help them maximise connectivity with the local community and partner agencies. You will provide holistic support ensuring that you support the person around any issues they may have, either directly or by signposting and referring to other services. You will work alongside them to build up basic living skills, as well as helping them integrate within their community.

You will be responsible for contributing to the targets set by Forward and the local commissioners. One of the key targets will be measuring outcomes, so co-operative working relationships with other partner agencies is vital to this role

Flexibility will be required in this role, this may include some evening/weekend shift and with reasonable notice to travel to other projects in your cluster to deliver services, enabling continuity of care for the client group.

- Take an active role in the implementation and achievement of project objectives
- Build strong relationships with, and makes full use of the support offered by, Line Management and Head Office
- Prioritise caseload depending on crisis intervention and severity of need and escalate to Team Leader as appropriate for support with prioritisation of the case

**Service Delivery**:

- Seek opportunities to be co-located with the District Housing Teams and Housing Providers for you to benefit from specialist housing expertise in supporting clients in treatment with a housing need.
- Provide preventative support for those in treatment that are entering or on a path to enter a housing crisis.
- Provide tenancy maintenance support for those in treatment that have recently moved into new accommodation to ensure they have support available to give them the best possible opportunity to maintain their tenancy.
- Provide wrap-around support in the practical, emotional and social aspects of preventing homelessness and maintaining a tenancy including;
- Budget Management
- Support engagement in treatment services
- Identification of challenges faced, e.g. financial, social, emotional, instances of domestic abuse etc.
- Social Integration
- Education, training and employment
- Linking to various third sector and local authority support
- Achieving specialist housing knowledge via co-location within the District Housing Teams and KCC commissioned training
- Coordinate and work alongside Health care to monitor and manage client health and minimise risks of harm.

**Performance Management**:

- Meet required KPI’s for corresponding deadlines, as set out by the Service.
- Plan and manage your caseload effectively, making sure that clients are prioritised appropriately and seen in a timely manner.
- Review ongoing care and treatment, liaising closely with healthcare and other professionals.
- Adopt a Reflective practice and make use of supervision to identify areas of strength and personal development.

**Departmental Management**:

- Proactively approach integrated ways of working support and adopt a multi
- disciplinary approach.
- Meet targets and deadlines in accordance with contractual needs.
- Contribute to the overall smooth running of the service by being proactive and solution focused.

**Quality and Safety**:

- Engage with regular service audits ensuring that any issues are escalated and addressed in a timely manner and good practice is effectively shared throughout the service to ensure continuous improvement.
- Alert management to any significant risks or problems arising in the course of managing and monitoring.
- Abide by all Forward’s policies and procedures and encourage others to do the same.
- Work with the highest regard to health, safety and security in the workplace, highlighting any risks (potential or actual) to the Service Manager and (where appropriate) the Head of Governance, in accordance with Forward’s risk management policies and procedures.
