**Background**
PKAVS is a charitable organisation that serves the community of Perth & Kinross by offering five community-led services supported by a team of 80 colleagues and up to 50 volunteers.
We operate across five locations in Perth & Kinross under our service banners which include Carers PKAVS, the Third Sector Interface, Mental Health, and Wellbeing PKAVS, Shopmobility PKAVS and the Big Hoose Project..
We offer these varied social care and welfare services as well as support and advocacy to Perth & Kinross' dynamic third sector.
At PKAVS our vision is a Perth and Kinross where people flourish, and communities thrive.
**Carers PKAVS** is our largest service. We provide support and respite care to unpaid carers aged 5+ across Perth & Kinross at our Carers Centre in Lewis Place Perth.
**The Third Sector Interface** is administered by PKAVS on behalf of the Scottish Government and provides four crucial support streams to the third sector in Perth & Kinross from the Gateway in Perth
- Voice - supporting the voice of the third sector.
- Connection - Connecting communities through the third sector.
- Building Capacity - supporting the development and growth of the third sector.
- Volunteering - developing and growing the volunteer programme to support the ongoing operation of third sector organisations.
**The Walled Garden & Wisecraft PKAVS** - provides therapeutic support and engagement activities that serve to improve the mental health and wellbeing of the people of Perth & Kinross from their two locations at the Walled Garden & Cafe in Perth and at our Wisecraft Workshop in Blairgowrie.
**Shopmobility PKAVS** promotes independence in the community by providing mobility support equipment from its purpose built Shopmobility hub in Perth's Canal St car park.
**The Big Hoose Project** supports children and families in Perth & Kinross living in poverty with access to free everyday household items such as toilet rolls, toiletries, cleaning products, bedding and more.
**The job**
We know that often individuals and families from minority ethnic communities and from cultural and social backgrounds that are harder to reach, may find it more challenging to access the services they need to support their lives and work. As part of our ambition to improve this situation in Perth & Kinross, we have an opportunity for a Community Outreach Worker to join The Walled Garden and Wisecraft team for a minimum period of 12 months.
Your role as Community Outreach Worker will be to engage with individuals and families in minority ethnic communities and other underrepresented groups. You’ll work directly with the communities to understand what support is needed and you’ll help to link these communities with activities available at The Walled Garden and Wisecraft and well as other professional and third sector support services working in Perth & Kinross.
**Duties and responsibilities**
The key role of the Community Outreach Worker is to connect individuals and families with the activities we provide at The Walled Garden and Wisecraft. Helping to identify outcomes that will promote their social, mental, and physical wellbeing. This role will assist in the transition into activities at The Walled Garden and Wisecraft; directly working with individuals in these activities for a short time, to help build confidence and build connections with other group members.
**Assessment and review**
- Identify those individuals, families, groups, and communities who are ‘hard to reach’ for example faith, cultural or community groups or those in remote and rural areas.
- Meet with, and build relationships with communities, individuals, and families in a variety of settings and analyse their chosen outcomes to help you understand if their support needs can be met by The Walled Garden and Wisecraft.
- Compete referral process into The Walled Garden and Wisecraft. Formulate and review support plans with individuals over a specific period, where the support reduces over time as people grow their knowledge, resilience and comfort within our service.
- Signpost or refer people to other services where appropriate.
**Community liaison and advocacy**
- Provide information, advice and support that will enable people to access activities at The Walled Garden and Wisecraft that will promote their physical, mental, and emotional wellbeing.
- Liaise with our Recovery and Development officers to ensure clients have access to appropriate support services, community resources and activities that meet individual outcomes.
- Connect underrepresented groups who do not wish to engage with The Walled Garden and Wisecraft to local social prescribers who can help navigate through the health and social care system; facilitating their access to appropriate services and support within the community.
