**Job description**
The CHAOS Group is seeking to hire a Community Gateway Locality Lead to join its ICB-funded, voluntary sector-led coalition. The lead will work to provide a simple point of access for everything the community has to offer in Cornwall, offer personalized and anticipatory care plans, prevent unnecessary hospital admission, reduce isolation and loneliness, prevent ill health, and minimize decline. They will help navigate, support, encourage and motivate people using coaching conversations, into connection and ongoing support networks, identify those often hidden or “waiting” for a service, and connect and work with social prescribers, GP practices, health and social care practitioners, and community networks. The role forms part of a single case management and recording system and will require regular reporting and monitoring of personal and collective impact and activity.

The Community Gateway is an ICB-funded, voluntary sector-led coalition. Its purpose is to provide a simple point of access for everything the community has to offer in Cornwall. It aims to offer as many adults as possible a personalised and anticipatory care plan. This is not a formal document or assessment; it is an approach that starts with a conversation focusing on what matters to them and develops into an active discussion led by the individual and supported by the Gateway locality lead.

It is a conversation that draws on the person’s strengths and assets to actively explore the self-management of their well-being within the context of their whole life, environment, and family situation. In doing so, the aim is to prevent unnecessary hospital admission, reduce isolation and loneliness, prevent ill health and minimise decline. Wherever possible, this approach supports the individual to gain the power to live the life they wish. The plan will draw on everything the voluntary sector and community offer, with the locality lead operating as an advocate for those less able to speak for themselves.

The Community Gateway is run by a collaboration of agencies, including Age UK Cornwall, The CHAOS Group, Cornwall Neighborhoods for Change, Cornwall Rural Community Council, Pentreath, and Volunteer Cornwall. It draws on the network of community hubs spreading the length and breadth of our county.

The locality roles form part of Gateway programme and operate within their ICA area.
Their focus is to dig into their communities, working with individuals, hubs, prescribers, health and social care practitioners, transfer of care hubs, to identify those who might best benefit from a personalised care and support plan. These roles will avoid duplication and instead identify those often hidden or “waiting” for a service; focusing on those more likely to decline due to a variety of circumstances; perhaps a recent trauma, diagnosis, vulnerability, a fall, a caring responsibility, a personal challenge. They will help navigate, support, encourage and motivate people using coaching conversations, into connection and ongoing support networks. They will become part of the foundation of their locality, working as part of the MDT/Huddle and ITOCH structure, advocating for those who struggle to have their voice heard - making the Gateway always the right place to come.

**Responsibility of Gateway Locality Host Organisations**
Each host organisation (CRCC, CHAOS and CN4C), are committed to hosting the roles as part of the wider coalition of Gateway partners. Roles are agnostic to individual organisation and reflect the shared values and principles of Gateway partners. There is a commitment to continual learning, coaching conversations, training and ensuring that personalised support plans, paperwork, reporting, paperwork, and communication are co-designed and consistent.

**Outputs and Outcomes**
The outputs for this role include:

- Forming part of the local ICA foundation.
- Helping to build an evidence base to demonstrate the power of codesign, voluntary sector support and locality-based coalitions; this role forms part of a single case management and recording system.
- Providing capacity and support to the Gateway service.
- Undertaking 5 guided conversations a week which may lead to personalised care and support plans.
- Connecting to other VCSE, volunteers and statutory locality teams.
- Connecting to community hubs to help identify activities and engagement that create a sustainable community-focused plan for individuals.
- Providing intelligence, ideas, and innovation to local discussions.
- Access a one-off small Personal Health Budget to support individuals in meeting their goals and as part of a personalised care plan.

The outcomes are harder to measure but are focused on supporting people to support themselves; improving the ability for people to make the choice that is relevant to them; improving well-being, and ultimately, reducing the risk of ill health, preventable disease, isolation and hospital admission.
