Clinical 1. To assess, plan and implement therapy to a designated caseload and adapt intervention to meet client needs. To progress and adapt designated therapy programmes in line with own scope of practice and competency set by the therapy team leaders (which will have been agreed at appraisal) and feedback to the responsible Therapist, taking into account changes in the patients health and social needs. This will include: Personal activities of daily living i.e.

washing, dressing, grooming, toileting Domestic activities of daily living i.e. meal and drink Preparation Following a programme of therapy set for individual patients on the caseload Mobility and transfer practice Assisting the patient with an exercise programme to improve their functional ability 2. To be responsible and proactive when organising caseload in discussion with the other team members. To ensure patients and carers are an integral part of their treatment planning and goal setting.

3. To be responsible for assessment, prescription, issuing and fitting of equipment, whilst demonstrating an awareness of local cost implications. 4. The post holder may be required to assess the level and urgency of therapy need, complete risk assessments and goals with families, individuals, and relevant professionals as required, to determine the level of assistance to be provided directly by the LIFE Service.

5. To use therapeutic handling skills which require dexterity and co-ordination, in the management of adults with acute illness and/or disability with varying mobility/balance/sensory impairment. 6. To work with patients individually or in a group in a variety of settings without direct supervision or presence of a qualified Therapist, including educating patients and carers.

7. To attend Multidisciplinary team meetings as part of the Home first service and/or the LIFE therapy community teams and pro-actively offer support/advice and/or accept referrals for patients. 8. The post holder will be required to provide interventions for non-complex patients within the Home first service and community LIFE therapy.

They will also support the therapist / senior nurse with other more complex patients within these parts of the service, providing verbal and/or written feedback to wider members of the team to support with management and planning of care, reablement and recovery and promotion of independence. 9. To liaise closely with the other members of the team regarding the patients progress, informing a senior therapist promptly of any change, deterioration or concerns in the patients health/needs. 10.

To communicate information to patients and/or carers, from a multicultural, ethnically diverse population where there may be medical barriers to their understanding. To understand the Physical and Psychological barriers to communication related to the patients condition and adapt communication according to the patients needs. (This may require the use of interpreters and/or non-verbal communication aids.) To undertake this with groups or individuals. 11.

To liaise with and work alongside other professionals / organisations, ensuring effective communication at all times in order to provide excellent patient care. 12. To comply with all Trust, Service and Professional standards, policies and procedures. 13.

To maintain a high personal standard of work and to seek advice from senior staff when appropriate. 14. To develop rapport with patients, encourage their interest and participation, in a manner, which promotes dignity, self-esteem and choice. This requires empathy, sensitivity and good interpersonal skills.

15. The post holder will keep accurate and up to date records of the person being supported in line with departmental standards and standards for record keeping. 16. The post holder will be required to use the Trust IT systems to enter the persons data, activity, service evaluation and any other relevant data collection.

17. The post holder will be required to participate in 1:1 and supervision arrangements organised by the LIFE Manager. 18. The post holder will be required to assess the level of need, complete risk assessments and support plans/ goals with families, individuals, and relevant professionals as required, to determine urgency of therapy need and the level of assistance to be provided directly by the LIFE Service.

19. The post holder will ensure that care plans are written, reviewed and accurate records maintained in the persons notes and office copies as required. 20. The post holder will escalate any issues or concerns in accordance with Safeguarding procedures and policy.

21. The post holder will work in a flexible manner in order to respond appropriately to the requirements of the organisation and the needs of the service, service users and patients over a 7-day period, working a shift-based rota within operating hours. 22.
