**Role Profile**
**Job Title** Dementia Support Worker (West Cornwall) 2 x Full-time posts available.
**Salary **£22,816 per annum (Band 3 NHS)
**Reports to** TBC
**Place of work** Office based in Hayle, with occasional travel for Memory Clinics across West Cornwall.
**Working hours** 37 Hours per week typically 9-5pm Mon-Thurs & 9-4.30pm Fridays.

**Role Purpose**
These roles are part of a new community dementia collaborative for Cornwall uniting the strengths of four organisations, Cornwall NHS Foundation Trust, Memory Matters, Alzheimer's Society and Disability Cornwall & Isles of Scilly, to provide comprehensive support to the community.
The Dementia Support Worker roles (DSW) aim to provide holistic and personalised support for people living with dementia and their carer/family to access what they need, at the right time, in terms of information, and connecting to services, activities, therapies, and practical assistance. The DSWs will work closely with the locality Primary care Dementia Practitioners, (working within the Primary Care Networks) assisting with their case load of people and connect and refer into community ensuring the right support, right place at the right time, with the aim of supporting people to remain in their own home and communities for as long as possible.
The post holders will facilitate choice and independence, through person-centered care & support to enable people to achieve the best possible outcomes and maintain their wellbeing.
**Activities and Responsibilities**
- Act as the primary point of contact for the Primary Care Dementia Practitioner (PCDP) and for people diagnosed with dementia and their caregivers following referral into the Dementia Support Service, in West Cornwall.
- Provide essential information, advice and guidance to people as appropriate and undertake referrals as needed, to community staff where required for 1:1 support and home visits.
- Help people navigate and access appropriate services, ensuring smooth transitions to other key workers within the Dementia Support Service such as Memory Café ‘Wayfinders’ ‘Dementia Advisers’ and Cognitive Behavioural therapy providers.
- Undertake formal holistic assessments with people to determine support required.
- Co-develop personalised care and support plans with people on your case list and their immediate caregivers, ensuring people living with dementia are involved in any decision-making processes. Cultivating therapeutic relationships with people you support.
- Undertake formal referrals to appropriate services as required whether internal or external.
- Aim to develop contact support systems every three months or so to check in and review peoples emerging needs as required.
- Escalate as necessary to PCDPs changes in condition or deterioration identified.
- Ensure accurate case notes are maintained and maintain safe standards of practice.
- Seek and record client feedback to ensure needs are met and plan necessary adjustments.
- Attend and support memory clinics as required by the PCDP, where assessing, diagnosing, and providing support for people experiencing memory issues, cognitive impairment, or suspected dementia take place.
- Promote and sustain great interpersonal relationships with stakeholders including partners, healthcare teams, voluntary sector agencies, etc.
- Maintain regular supervision and stay informed about dementia and related physical and emotional health issues.
- Develop networks with organisations and contribute to an evolving database of local information and sources of activity and support.
This role demands compassion, strong communication skills, written and verbal, and the ability to work effectively within a multidisciplinary team to ensure the wellbeing and the development of care and support pathways for people living with dementia and their caregivers.
