JOB SUMMARY To provide clinical and managerial leadership for an allocated team of nursing staff in the development of therapeutic evidence based culture which maximises patients rehabilitation potential, whilst maintaining a safe and secure environment. To support and deputise for the Unit Manager in the running of the ward area/s ensuring the highest possible standards of nursing care are carried out in accordance with the policies and procedures of the Unit and Trust. KEY RESULT AREAS: Clinical You will support the Unit Manager in contributing to the development of high quality patient focused care, acting as a role model to junior staff and demonstrating clinical expertise. This will be achieved by - 1.

Responsible for creating and maintaining a high standard of quality of care throughout the Unit and addressing any concerns not meeting these standards proactively. 2. Engage with service users in an empathetic positive manner that respects individuals Psychological, Social, Physical and Spiritual needs. 3.

Ensuring that care plans developed by ward staff are completed fully, monitored and adapted in light of changing service user/carers needs and completed in negotiation with the service user/carer and validated by the service users/carers signature. 4. Ensuring named nurse/key workers undertake assessments, planning, implementation and evaluation of care plans and maintain adequate and accurate service user records. 5.

To appraise MDT members of allocated service users progress and support junior members of staff in their contribution to the MDT process. 6. Ensure that effective communication is maintained with the nursing team, MDT members, service users and carers through verbal and written channels. 7.

Ensure that the CPA process is effectively implemented and monitored in the team and that corrective action is taken when necessary. 8. Providing a highly effective, visible, approachable presence on the ward. 9.

Having an in-depth, up to date knowledge of clinical developments relevant to Low Secure mental health nursing. 10. Contributing to quality initiatives and clinical guidelines on the unit, promoting research, audit, innovation and standards that are evidence based. 11.

Acting as named nurse/key worker in the key workers absence. 12. Ensuring that named nurse/key workers complete risk assessments, rating scales and devise appropriate risk management plans in accordance with local policy. 13.

Ensuring the highest level of clinical knowledge regarding individuals on the ward to ensure effective decision making. 14. Being involved in developing systems with the Unit Manager and where appropriate taking the project lead in monitoring the efficacy of the systems. 15.

Ensuring all nursing staff actively engage with service users and carers, where possible involving service users and carers in decisions regarding their care and encouraging their involvement in service development. Ensuring all staff are aware of customer relation issues and engage where necessary with all clients, carers, relatives and visitors in a professional manner to a high standard.
