The role of Care Co-ordinator is an administrative post, pivotal in bringing services together in order to provide seamless care for people within West Norfolk. The service provides core functions - acute hospital avoidance and administrative facilitation of Virtual Ward care service.

This is an exciting opportunity within the Virtual Ward coordinator service for a reliable and committed person who is passionate about health and social care to take on a challenging and sometimes pressurised role.

You will need to have excellent organisation skills and be a skilled communicator, as the role is integral to the development of integrated and co-ordinated services. A good level of IT experience/computer skills is required as managing data on a daily basis will be required.
- Receive referrals from a number of agencies. Allocation and coordinate of palliative, end of life and intermediate care packages.
- Maintain and develop database monitoring
- To be responsible for maintaining service users’ rotas
- Discuss any relevant matters with families and carers to support good communication and care delivery
- Recording patient information on systmone
- To carry out a baseline risk assessment in conjunction with the referring agency
- Conduct a needs analysis in conjunction with District Nurses, Social Services, etc to assess and develop the necessary care packages
- Be responsible for ordering and monitoring stock level for the service
- Be flexible in approach to duties and receptive to change
- Attend in-service training programmes and statutory lectures when directed
- Taking and recording accurately telephone messages and answer-phone messages
- Maintain data as required by the service
- To organise staff attendance at relevant training courses
- To act up for the Care Coordinator during periods of absence (e.g. annual leave, sick leave)

Applicants must have the ability to recognise the importance of confidentiality and sensitivity of issues, acting with discretion at all times and be flexible, calm and organised whilst meeting the high demands of the post.

You would need to have previous experience in a health or social care environment (or similar) and have excellent administration skills to communicate between primary care, community services and hospitals (physical and mental health), local authority, acute trusts and voluntary services.

The hours of work will be 37.5 over 7 days which include weekends and bank holidays.
- To be responsible for taking telephone calls, allocating and organising staff to visit patient’s homes. Supporting the work of the Care Coordinator and working alongside the integrated teams and a verity of Organisations and Care providers.
- To be responsible for supporting Registered nurses in allocating patient visits and communicating with the wider organisation.
- Engage with mobile working
- Engage with SystmOne Optimisation
- Engage with referral to discharge standard processes
- To be aware of the demand and capacity model which will reflect workload needs at any given time
- Work in a flexible manner
