**This is the type of job that you will give you that fuzzy tummy feeling**

You will be supporting vulnerable single adults who are homeless or pending homelessness and have support needs. The service provides them with accommodation and support to get them to full independence.

Your role will be to work with the service user to establish an agreed support plan to encourage independence.

**Ideal specifications**:

- Have knowledge or experience of social care practices.
- Good communication skills.
- Empathetic and understanding nature.
- Good understanding of equality and diversity guidelines.
- Able to follow good working practices and procedures.
- Have knowledge and understanding of writing care plans and risk assessments.
- On occasion your tasks may require you to work outside of your normal hours of work.

**Role**:
- Provide support to vulnerable homeless individuals who have identified needs.
- Together with the service user develop a comprehensive and personalised support plan that will highlight their needs and include an action plan to aid them in achieving goals that they have agreed.
- You will be expected to highlight further needs if they arise whilst the service user is accommodated within the service.
- Ensure that a risk assessment has been completed and that you have the information necessary to support the individual appropriately.
- Induction - into the accommodation and the service by accompanying them to the allocated property and introducing them to other service users that they will be residing with.
- Conduct regular key work sessions with the service users allocated to you ensuring that you provide them with the opportunity to add to the agenda for the key work meeting.
- Keep legible and comprehensive key work notes and other reports of your contact with your service users.
- File any such notes accurately and keep all service users information confidential.
- Ensure that the service user is engaging with relevant community services to aid in their progression.
- Ensure that you keep in touch with community services that are working with your service user to keep informed of their progression, and intervene if you are informed that they are not engaging.
- Implement further measures keep them engaged where necessary.
- Ensure that they are aware of the conditions of their licence and the house rules/health and safety and support them as much as practicable to refrain from breaching.
- You will be reminding them of their obligation to pay service charge so that they do not go into arrears.
- You will be expected to advise your service user on life skills and other topics that may aid them in their recovery and moving on.
- You will hold regular key work sessions, house meetings, workshops - and update records.
- You will be expected to work with the other teams to ensure the safeguarding of the service users.
- Other relevant matters that will ensure that the service provided is of a high standard and considering the dignity privacy and respect of the client.
