To play a pivotal role in the delivery of high quality primary health care within the Primary Care Network. Work closely with OneNorwich practices, practice managers across Norwich and staff within the PCN to understand current services models, identify challenges, agree priorities for improvement and the support these to be delivered. Monitor progress on the implementation of programmes within the PCN transformation and continuous improvement plan and provide assurances to the Clinical Directors, PCN Managing Director and the executive leadership team that the envisaged benefits from programmes are being realised. Develop strong relationships with Norwich PCN practices and will support the planning and delivery of improvements that will provide benefits to patients and staff.

Ensure all PCN contractual obligations of the practices are structured in order that all reports and technical reporting are robust. Identify best practice and promote the most efficient ways of collaborative working through Digital platforms. Operate effectively in a flexible and demanding environment and proactively engage with all stakeholders working on various topics including national programmes. Support practices to increase their patients participation of digital services, for example online access to ordering medication.

Demonstrate clear and concise management with Information Governance. To commit to working and engaging constructively with internal and external stakeholders on a range of contentious issues.

To utilise influencing and persuasion skills to establish the basis for the successful delivery of service development and transformation workstreams. Implement and maintain websites and social media platforms. Be an advocate for continuous improvement facilitating & sharing good practice & collaborative working across Norwich and the wider Norfolk and Waveney area in all that you do. The post holder will develop relationships with Norfolk and Waveney ICB to discuss and agree local transformation plans and activity, and to support alignment with broader strategy, including the Norfolk and Waveney ICB/ICS digital strategy.
