Our care client who work with Domiciliary Care and Extra Care are looking for an experienced Care Coordinator to join the Team in their office near Aylesbury HP10 8DP office
**As a Care Coordinator you will need to have/hold.....**
- You must currently hold or be prepared to work towards your Social Care NVQ/QCF Level 3 Qualification.
- Experience of working with Adults that fall into the categories of Older People / Dementia / Physical Disability / Sensory Impairment / Learning Disability
- Car Driver

**Responsibilities of the Care Coordinator**
- Ensure that the Quality of Service Provision meets with policies and expectations.
- Ensure that Regulatory Requirements are met.
- Implement a high quality service through the effective matching of service user needs with suitably qualified and experienced care workers.
- Facilitate effective communication between the Organisation, service users, care workers and outside Health Professionals and Local Authorities.
- Ensure a high level of records are imported and held on HCR for both Care Workers and Service Users.
- Manage and monitor the branch’s rostering requirements of its care team
- To ensure all aspects of health and safety requirements are adhered to as specified within the Health and Safety Policy and Procedure [HR02] and in accordance with training given.
- At all times observe and respect the service user’s dignity, privacy and independence as far as is practical.
**Key Tasks of the Care Coordinator**
- Maintain accurate, concise, timely and legible records relating to all service aspects
- Identify a Care worker with appropriate knowledge, skills and attitude to match the needs of the service user
- Follow up the initial contact between the care worker and service user to evaluate the Care Plan and ongoing needs
- Undertake telephone risk assessments, monitoring of service user’s satisfaction and care worker performance
- Liase with appropriate professionals and the Organisation’s management as required.
- Maintain accurate written and computerised record keeping
- Cover all calls on HCR and ensure weekly rotas are sent out to staff by Thursday afternoon.To effectively manage the branch’s unallocated and permanently unscheduled visits to a compliant standard
- Manage the allocation of travel time between individual visits
- Coordinating hospital discharges
- Support the Branch Manager with monitoring of staff through one-to-one supervisions and investigation meetings
- With the involvement of the Branch Manager address and deal with complaints / concerns raised by service users, family members and health professionals
- Participate in staff, team and quality management review meetings as directed by the Branch Manager / Operations Manager
- Report back to the Branch Manager / Operations Manager on any aspect of service user care which warrants investigation or urgent action
- Be aware of the tasks and activities which must NOT be undertaken as part of care duties, as set out in relevant Policies and Procedures
- Undertake on-call duties as directed and record/report all on-call activity to the Office
- Allocate all unassigned calls during on-call hours - where visits cannot be assigned, undertake the visit
- To liaise closely with colleagues and other care professionals
- Line Managing staff through conducting one-one supervisions, spot checks and annual appraisals
- Maintain confidentiality of information at all times
- Safe running of the branch in the absence of the Branch / Deputy Manager
- Any other such duties required to ensure the safe running of the services and delivery of appropriate care to service users
- Provide care and support to service users at times required; both during and outside office hours
- Carry out any other task reasonably requested by the Branch Manager
