Reporting to the Head of Patient Safety, Concerns & Learning this is a new role created as part of an organisational change process which has seen investment across the whole team. This role provides an exciting opportunity to influence patient safety both internally and health system wide, forging effective relationships across NHS Wales and wider. The Deputy Head of Patient Safety will lead the operational functions of the Patient Safety Team putting patients, service users and families at the centre of everything we do. Ensuring the Trust continually attains the requirements set out in existing and new legislation and regulations and commissioning frameworks will be a key function of the role.

This exciting new role is a leadership position with a purpose to ensure that Trust meets requirements of the Putting Things Right Regulations, Duty of Quality and Duty of Candour.

Communication and Relationships

Ensure that all leaders, managers, and staff have accessible competent patient safety advice and guidance, ensuring compliance with national and local guidance or protocols.

Will establish effective two way communications, engaging the workforce in the promotion and achievement of an improving patient safety culture and organisational learning.

Represent the Trust in attending high level external meetings / events e.g. NHS Wales Executive, Welsh Government, Healthcare Inspectorate Wales, UK wide events and Coroners Court as required.

The post holder will be required to provide professional advice and guidance on the development of relevant Trust strategy and policy planning matters from a patient safety and organisational learning perspective.

Provide advice and support to Directorates across the Trust on elements of patient safety. Leadership and influence is required to inform Local, Directorate, and Corporate Risk Registers.

Develop and maintain close working relationships with relevant external bodies including Health Care Inspectorate Wales, Welsh Risk Pool, NHS Wales Executive, Welsh Government, Health Boards and other external agencies, to support an integrated service.

Act as a senior point of contact for patient safety and concerns issues.

Act as an ambassador for the Trust at regional and UK national levels through membership of Association of Ambulance chief Executives’ (AACE) Quality Governance and Risk Directors (QGARD) sub-group and associated forums.

Develop and maintain internal relationships with others responsible for specialist areas of patient safety related activity to ensure the effective exchange of ideas, information and learning. For example, Health & Safety, Infection Prevention and Control, Clinical Audit, Safeguarding, Patient Experience and Community Engagement, Quality Improvement, Quality Assurance and Training & Development.

Analytical and Judgement

Responsible for managing an assigned patient safety work programme, which systematically recognises reports, analyses, evaluates and minimises patient safety risks throughout the organisation. Monitor progress and manage any intervention required to ensure successful delivery of the work programme.

Support external investigations or inspections by Health Care Inspectorate Wales, Local Government Bodies and other relevant bodies, ensuring collaboration and transparency.

Undertake the training needs analysis (TNA) for patient safety, identifying gaps in patient safety training in the Trust. Responsible for quality assuring the delivery of these training packages to ensure adequate standards of training are implemented.

Undertake the investigation of Nationally Reportable Incidents (serious incidents), notifying the NHS Wales Executive and Healthcare Inspectorate Wales (HIW) and Health Boards (as required). Undertake root cause analysis for serious incident investigations.

To notify the Head of Patient Safety, Concerns & Learning of any perceived patient safety concerns, recommending appropriate management action.

Scrutinise and develop patient safety data / dashboards including inspection reports and Datix Cymru to identify themes and trends, translating these into organisational learning and development.
