The Clinical Directorate are delighted to be recruiting two Clinical Leads for Integrated Care, Clinical Support Desk (CSD). The position will provide strong professional clinical leadership for Health Professionals and other allied clinical colleagues working within either the Clinical Contact Centres (CCC) or remote working locations.

The Clinical Lead Integrated Care (CSD) position will have a key role in the clinical development and delivery of Integrated Care, contributing to the overall operational and strategic development of the Trust to deliver clinical services of the highest possible standards. The Clinical Lead, Integrated Care (CSD) will possess significant post-registration experience.

The individual will ensure that agreed systems are in place to ensure a strong clinical presence within Integrated Care (CSD) and enable to the Trust to operate to the required professional clinical standards.

The post holder will work in partnership with senior management teams, Heads of Operations and Practice Placed Education Teams to ensure the development and maintenance of sound clinical performance management arrangements and clinical performance measurement systems. Along with the Consultant Clinician for Integrated Care (CSD), they will be accountable for the improvement of the requisite clinical metrics determined by the organisation.

The role will primarily be remote working however there will be a requirement to visit the CCC.
- Undertake an active clinical and leadership role in line within their scope of practice and job plan, ensuring that safe and effective care is delivered to the highest standard.
- Collaborate across care and emergency service sectors and in partnership with other professionals, to implement approved evidence-based pathways, procedures, and clinical guidelines in line with strategic policy.
- Provide the local engagement for clinical advice and support to practitioners in relation to clinical decision making through face to face and remote assessment, triage and clinical contact.
- Engage in the clinical and leadership agenda in support of operations and personnel, to contribute to safe working practice standards.
- Evaluate relevant information, research, patient surveys and clinical cases in ensuring improvement to implement treatment and management strategies in Integrated Care (CSD) with the aim of meeting patient, family and community needs which reflect the evidence base.

The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
