**About the role...**
Working with the Intensive Housing Support Team supporting individuals aged 16 plus, who have long and enduring mental health. The role involves providing housing related support, including Pre-tenancy support, preventing homelessness and supporting to maintain a tenancy. The role also involves working closely with individuals, supporting them to access support for their mental health and working closely with mental health services. In addition the team offer meaningful activities group work sessions, which is on a rota between the team.-
**Job roles include**:

- Support to access clinical gender identity services
- Peer support groups and activities
- Family support
- Sexual health guidance and signposting
- Drugs/alcohol brief interventions
- Signposting/support to access partner agencies/specialist services
