An exciting opportunity has arisen to work with our friendly team at Prime Home Care as a Care Coordinator. We are an established home care and domiciliary care provider company within MMAB CONSULTING LTD.

This is a full-time position based in our office in Darlington, Durham area.

Duties will include:

- Managing the carers rota
- Accepting, allocating and processing new care referrals
- Writing person-centred care plans.
- Organising the senior care team to complete monitoring calls
- Liaising with clients, their families, social services, and other partners to ensure safe, caring, person-centred care is consistently delivered.
- Ensuring carers are supported in a friendly and organised environment
- Assisting the manager with recruitment
- Taking part in out-of-hours on-call rota
- Ensuring accidents and incidents are recorded, reported, and resolved
- Carrying out general office duties
- Working flexibly as part of the team, ensuring all policies and procedures are followed.
- Work experience in the Domiciliary Care industry.
- Experience as a care coordinator or preferred but optional. Senior carers looking for their next challenge will be considered.
- Able to use initiative and suggest new ideas and change hands-on in approach to work
- Ability to work well under pressure
- Calm and friendly personality
- Excellent organisational skills
- Good understanding of Microsoft Word, Excel and Outlook
- Completed or willing to work towards a Level 3 Health and Social diploma driving license and access to a car would be preferred

Salary: £20k-£25k per year
