Advanced Clinical Practitioner

We are a dynamic multi-disciplinary team in Liverpool Orthopaedic and Trauma service (LOTs) and we have an exciting opportunity for an experienced Advanced Clinical Practitioner to join our team.
We are a regional Major Trauma Centre, we have an acute Neck of Femur Unit, acute Orthopaedic Wards with 100 inpatient beds as well as Fracture Clinic and virtual fracture clinic at the Aintree Site.
We are seeking a highly-motivated individual who is committed to providing excellent independent clinical practice, whilst demonstrating a willingness to share their extensive knowledge / experience with the wider clinical team. We have recently recruited to a trainee ACP role and are seeking to grow and develop our service.

The post holder will practice as an autonomous Advanced Clinical Practitioner within the Care Group. Working alongside senior medical team, the post holder will examine, assess, diagnose and treat patients within their scope of professional practice.
The job is practically based, intended to ensure skills and knowledge are related to high quality care and efficient service delivery, including achievement of national targets and best practice tariffs.
They will use their clinical skills and competencies to support holistic patient care and treatment.
They will abide by the four pillars of clinical practice and ensure they follow good governance processes in their practice.
Liaising with members of the MDT including the medical, surgical, nursing and therapy teams to ensure great care delivery for patients.
Communicate effectively plans of care for patients and with families as well as using excellent documentation to evidence practice.
Support the safe and effective pre and post operative patient journey and advocate for patients in their care.

The post holder will act, at all times in accordance with their Professional Code of conduct and Trust Policies.
Clinical Responsibilities
**The post holder will**:
Contribute to the development of an excellent clinical environment which is conducive to delivering high quality patient care and the achievement of excellence in service delivery.
Work as an autonomous practitioner within trauma and orthopaedics demonstrating advanced clinical knowledge and skills in relation to the assessment and management of patients.
Undertake full clinical assessment and examination of patients, providing a detailed medical history.
Order/perform investigations and diagnostic tests as required.
Interpret test results, where possible diagnose and initiate treatment plan.
Refer patients appropriately to medical staff/MDT as required.
Effectively communicate with GPs and other members of the Primary Care Team.
Assist in the production of discharge summaries.
Ensure follow up appointments/arrangements are made.
Actively participate in the development of care pathways, guidelines and protocols as appropriate.
Interview and support patients and their relatives.
Educational Responsibilities

Participate in internal training and education programmes
Promote and display evidence of development in clinical practice and related clinical research which further enhance the acumen and level of professional knowledge.
Ensure that own practice and that of others within the department is consistent with current research knowledge and is evidence based.
Establish and develop an information resource for patients and their relatives.
Actively support trainee ACPs in the service to develop.
Innovate and promote new ways of working that support patient care and great patient experience.
