We are looking for an Independence Co-ordinator to join the Independence & Reablement team based in the Selby area.

You will work alongside the OT’s and the Reablement Manager to work with people in the community to attain maximum independence within the choice of lifestyle, through the provision of simple assessment and non-complex equipment, adaptations, and Telecare solutions.

The core focus of the role is preventing, reducing and delaying the needs of adults with care and support needs by promoting independence and recovery.

The aim of the team is to support adults with care and support needs, and carers, to maximise their independence, regain skills and confidence in particular tasks or activities of daily living.

Your role will be part of the delivery function of the teams, which is undertaken by the Reablement Care & Support Workers and Independence Co-ordinators, and includes the provision of simple equipment, offering people information, advice, and guidance, or commissioning a package of support for a short period to aid the person’s recovery.

The Independence Co-ordinator will work across the Care and Support pathway, ensuring that simple and less complex equipment and adaptations are delivered, fitted, and demonstrated to adults with care and support needs, and their Carers.

You will also be responsible for the review of simple and less complex equipment that sits within the caseload of the Independence and Reablement Service.

IT skills are essential for the role.

A certificate in health & social level 2 is essential.

Working hours to be 18.5 hours, with weekend working included on a rota basis.

There is an expectation that you will be available to work as necessary outside of office hours.

A DBS check is required for this role.
