Community Health Care Worker

Overview:
We are seeking a dedicated and compassionate Community Health Worker to join our friendly team. As a Community Health Worker, you will play a vital role in providing support and assistance to individuals in need of healthcare services. Your primary responsibility will be to promote health and wellness within the community by providing care according to care plans and risk assessments.
**Responsibilities**:
- To work as part of a friendly team in a family based setting
- To provide care in line with care plans risk assessments and best practice
- Maintain accurate documentation of client interactions and progress
- Communicate effectively with clients, families, and healthcare providers
- Assist with transportation to medical appointments or community resources as needed
- Utilize technology and IT systems to record and track client information
Qualifications:
- High school diploma or equivalent required; additional education or certification in healthcare or social services is a plus however full training is provided
- Previous experience in a similar role, such as home care or community outreach, is preferred
- Strong communication skills, both verbal and written, with proficiency in English
- Compassionate and empathetic approach to working with individuals from diverse backgrounds
- Ability to work independently and as part of a team
- Valid driver's license with reliable transportation
- Proficiency in basic computer skills, including the use of electronic health records
**Salary**: £11.50-£15.00 per hour
