ESSD Rehabilitation Assistant (band 3) based at Hilltop Heights in Carlisle - 30 hours.

The Early Supported Stroke Discharge (ESSD) service is excited to advertise a post for 30 permanent hours for a stroke-specific Rehabilitation Assistant to work within the Carlisle localities. Our team is based at Hilltop Heights in Carlisle and you will be working with people in their own homes as well as across multiple NHS & community sites depending on the needs of the stroke survivor. The ESSD service plays a vital role in safe, supported & early discharge from the Hyper-acute Stroke Unit & the stroke ward at the Cumberland Infirmary & we often work with our stroke survivors before their discharge from hospital. As a Rehabilitation Assistant in the team you will be pivotal in the implementation of evidence based specialist treatment programs for patients following a stroke who may have a range of impairments and complex health needs. The therapy plans often include supporting patients to practice their normal daily activities such as personal care, meal preparation & support with medication in order to achieve their rehabilitation goals. You will work closely with patients and their families and carers in the implementation of their care plans. You will be working with a full multidisciplinary team comprising of specialist occupational therapists, physiotherapists, speech and language therapists and neuropsychologists.

The ESSD service works closely with the Integrated Care Community teams to provide a seamless transition of care from the acute hospital setting into the community. This will require the ability to travel across the Eden & Carlisle localities as required, as well as to work flexibility to meet service needs. You will provide specialist rehabilitation with patients following a stroke, in their own homes & other community & clinical settings, and under the direction and supervision of qualified therapists. The post will involve weekend working.
