We have a fabulous opportunity for two support coordinators to join our awesome team. You will be working with customers who are experiencing homelessness (often street homeless) and are referred to us with multiple complex support needs such as poor mental health and coping skills, as well as substance or alcohol misuse and other behaviours.

We support our customers with their goals to gain more independence as well as build resilience to help improve their outcomes ready for their move on into their own accommodation in the community.

Our customers live in a supported housing service in their own fully self-contained flat and have access to communal facilities within the service. You would be primarily based in the office at the service where we have facilities to assist us in a mission to help and support some of the most vulnerable persons in Gateshead. We help our customers achieve goals around their housing and other support needs by working in a person-centred way to help promote independent tenancy skills as well improve their health, wellbeing and resilience.

**About you**

You will be passionate and motivated to help vulnerable people make positive and lasting changes to their lives. You’ll impress us with your ability to work on your own initiative, positively influencing people from all different backgrounds.

You’ll help change our customer’s lives (and our services) for the better by undertaking risk assessments, support planning, goal setting as well as regular reviews. You’ll create bespoke and personalised support plans with and for your customers. Each plan will look to meet our customer’s individual needs and aspirations, making sure they meet safeguarding and confidentiality requirements. You’ll also have responsibility for coordinating our support workers, apprentices, and volunteers, and working collaboratively with external agencies, to assess and engage support for our customers.

To make sure our customers are well supported, it’s important that you have worked with Vulnerable adults. Don’t worry, we won’t drop you in the deep end, we’ll help to you understand how things work at Home Group

To make sure that we keep our records straight, and customers safe, it's really important that you are technologically savvy (or at the very least not a technophobe) You’ll be given the latest IT kit, which could include a Surface Pro tablet and smart phone, supported by our new online integrated IT system which means that you don’t have to be in the office and can work as you go.

As this is a community based role, you may need to travel around to spend time with your customers, this may be using public transport, or if you drive and have access to a car that would also be amazing (we pay your expenses to cover this use)

**About our team**

At the service you will meet staff who have a wealth of Home Group knowledge and intermingle with colleagues from all different departments with different specialities.

**Working hours**

You’ll work Monday - Friday on a rota basis between the hours of 8am and 8pm so the weekend will be your own The rota was put together by the support team and gives a great work life balance.

We need you to be flexible to meet the needs of our customers, but we aim to give you the time you need to spend time doing the things that matter to you

**Bank**

Looking for a more flexible working pattern? You can also join us on a flexible basis by being part of our bank. You can work hours that fit around your current commitments or even pick up some additional shifts to your main role to earn some extra cash and get paid on a weekly basis.
- When you join, you’ll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you don’t have a transferrable one._
