An exciting opportunity to work within the Lung Cancer CNS team in a large Lung cancer directorate at Wythenshawe hospital. The role will involve working alongside and supporting the CNS team in providing care and support to lung cancer patients and their families across the whole cancer pathway from diagnostic tests through treatments to survivorship or supportive care. The postholder will have specific responsibilities in relation to triaging calls, and supporting the national personalized care programme, alongside working closely with the CNS team to ensure delivery of best care for this patient group.

2 x 37.5 hour per week positions available

The post holder will work as part of the Manchester University NHS Foundation Trust Lung Clinical Nurse Specialist Team to support the CNS team in delivering specialist nursing care to lung cancer patients and to support delivery of the nationally recommended Personalised Cancer Care Programme (Holistic Needs Assessment & Care Planning, Health and Well-being Events and Treatment Summary) including: -.

Providing support to the lung cancer nurse specialists in delivering care and support throughout the lung cancer pathway including diagnostics/surgery/oncology treatments/survivorship/best supportive care.

Providing support to cancer nurse specialists implementing personalised cancer care including assisting with holistic needs assessments and treatment summaries and organising health and well-being events.

Communicating effectively (daily and directly, face-to-face and telephone) with a range of individuals on various levels, eg: - patients, relatives, carers, other health / social care professionals and community workers.

Providing general information and support about cancer and cancer services to enable navigation of health and social care and to make choices that are best for the individual’s cancer and their life.

Promoting equity for patients across the Greater Manchester footprint.

The post holder will be expected to work with all grades of medical & nursing staff and wider health / social care and community teams (including third sector and charity).
