Rehabilitation Assistant

We are looking to recruit a senior Rehabilitation Assistant. Your role will be to assist the multiple-disciplinary team in the day to day management and rehabilitation of acute stroke patients. You will work closely with the occupational therapists, physiotherapists, nurses and the clinical psychologists.
You will be based in the Stroke Pathway assessment and rehabilitation Centre at Beech Hill.
You will undertake assessments under the guidance of qualified staff, implement and progress rehabilitation programs, set and review Goals and at times provide Specialised enabling care support.
You will be responsible for the delivery of high standards of patient care and the efficient flow of patients through our service.
You will be working at SPARC which is the Rehabilitation centre of the Integrated Stroke Pathway. The patients are acute stroke patients needing further rehabilitation before being discharged to the community.
You will be supported to increase your skills and knowledge and work closely with the integrated stroke pathway.
You will be qualified to NVQ level 3 In Health/ Social Care or equivalent and have significant experience of working therapy assistant or generic care support worker.
You will also have had experience delivering excellent patient care and of liaising with clinical staff to agree care levels/ requirements.

Carry a clinical caseload delegated by Occupational Therapists and Physiotherapists, plan and prioritise own work and report back to qualified therapy staff.
Assess, treat, monitor and progress each patient within protocols.
Provide treatment in hospital and/or in the community,
Conduct an initial assessment following protocol on patients delegated by Occupational Therapists and Physiotherapists.
Set up equipment for therapy and ensure it is safely used by patients, carers, staff and/or students.
Teach, guide and supervise patients as individuals or in groups.
Select treatment activities to fulfil therapy goals, taking into consideration the patient’s condition and capability.
Carry out clinical duties that contribute towards integrated patient care as part of therapeutic practice. These would be supported by a training package and competency development.
Complete assessment forms and write in patient treatment records in line with Trust and Therapy Services local guidelines.
Conduct discharge planning and ongoing referrals to other agencies within protocol.
May be asked to work across the integrated stroke pathway HASU,ASU,SPARC, CSS.
