Health Improvement Practitioner

We are seeking to appoint an enthusiastic and well motivated professional to join our Public Health Team in Cwm Taf Morgannwg working across the three counties of Bridgend, Merthyr Tydfil and Rhondda Cynon Taf.
The post holder will contribute to the development, implementation and evaluation of local health improvement programmes/projects. Implementation will be system-wide, engaging health, local authority, third sector colleagues as well as wider partners.
The post holder will also provide support to the delivery of health improvement programmes/projects through; sharing relevant public health information, engaging partners in local approaches to improve population health, regularly facilitating training courses and engagement sessions as well as undertaking routine monitoring and evaluation of programme/project activity.
A specific focus will be to take forward health improvement programmes and initiatives e.g. Making Every Contact Count approach and to liaise with partners to influence the adoption of preventative approaches to improve population health.
The post holder will be part of the Public Health team in Cwm Taf Morgannwg working across the three counties of Bridgend, Merthyr Tydfil and Rhondda Cynon Taf.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

**The post holder will**:
Support the team in identifying and developing collaborative partnerships with organisations working across the system maintaining solid communication links with identified contacts.
Work across organisations to take forward health improvement programmes and initiatives e.g. Making Every Contact Count approach.
Liaise with Cwm Taf Morgannwg Health Board (CTMUHB) staff and external partners to influence the adoption of preventative approaches to improve population health.
Development and maintenance of a directory of contacts from across the system, in line with data protection legislation.
Develop methods of routine communication including e-bulletins/use of social media to reinforce/share key public health messages and approaches in conjunction with the team.
Identify and disseminate information on key public health messages, initiatives and good practice across all partner organisations.
Routinely facilitate training for staff and volunteers within organisations across the system on health improvement messages and approaches, across a range of public health areas and using a range of styles.
Utilise knowledge of behaviour change approaches when working with organisations and facilitating training.
Support the team in the identification of health data as part of health improvement programme planning and implementation.
Work with the team to regularly reflect on the effectiveness of health improvement programme delivery, to ensure that a culture of continual improvement is development and maintained.
Participate in reflective practice as part of ongoing personal development.
Organise/participate in local health improvement events/ campaigns in line with local needs.
Manage the design, development and content of relevant training packages in conjunction with the team, ensuring key messages and approaches remain up to date and evidence based.
Co-ordinate day-to-day health improvement project activity e.g. arranging training, maintaining training calendar, organising meetings and local events guiding the work of those providing administration support.
Provide health improvement advice which supports and enables others to take a preventative approach in their care/education of patients/clients to improve health.
Work closely with team members to support the development and implementation of health improvement programmes and projects.
Contribute to the development of systems and protocols required to enable and support programme/project delivery.
Take account of any relevant local and national policy/strategy developments and their implications in terms of local health improvement programme development.
Maintain levels of relevant stock, equipment and resources required to support health improvement programme implementation.
Support/guide the work of those undertaking administration support for health improvement programmes.
Move equipment and resources to locations across Cwm Taf Morgannwg when facilitating training/participating in health events.
