**Job overview**:

- Provide support to the Palliative Care meetings including recording actions and outcomes and national data collection, and other data collection identified by the MDT.
- Provide full assistance to the Palliative Care Clinical Team in the co-ordination of the Multi-Disciplinary Team Meetings and representing the MDT Teams to disseminate the information regarding the patient pathway.

**Main duties of the job**:

- Communicate effectively with all service teams, Senior Clinicians and other Trust Senior Managers.
- Provide support to the Palliative Care meetings including recording actions and outcomes and national data collection, and other data collection identified by the MDT.
- To persistently track, update and upload all the data referring to referrals and update the electronic Tracking Records, Specialist or local MDT Teams.
- Provide full assistance to the Palliative Care Clinical Team in the co-ordination of the Multi-Disciplinary Team Meetings and representing the MDT Teams to disseminate the information regarding the patient pathway.
- Provide timelines that clearly document the patient pathway for the Palliative Care Meetings and Directorate Meetings.
- Provide cross cover support to other members of the Admin Team for annual leave, sickness and other business needs as identified by management team.
- To be responsible for the timely typing of all correspondence inc. letters or reports as appropriate. It will be necessary to work to deadlines and be adaptable to last minute changes.
- Must demonstrate good communication skills and be able to sensitively speak to patients and their relatives on the telephone.

The Hospital Specialist Palliative Care Team provides:

- Direct patient care where palliative care needs are complex and cannot be met by the patients treating hospital team
- Indirect patient care through advice and education of generalist colleagues (including the development and implementation of patient pathways).
- The palliative care team provides emotional support for patients and families during sometimes very distressing situations. You will need to demonstrate good communication skills in order to sensitively speak to patients and relatives on the telephone.

**Detailed job description and main responsibilities**:

- The MDT Coordinator will be responsible for facilitating the weekly meeting(s) duties
- Post holder will ensure that the list and outcomes of the MDT meeting are disseminated to all the identified care providers and that all relevant treatment plans are documented in accordance with Trust policy for future reference
- Proactively manage telephone calls some of which could be of a sensitive nature from healthcare professionals internally and externally whilst maintaining a high level of patient confidentiality
- To communicate the details of the MDT meetings to all relevant stakeholders.
- To be responsible for the taking and production of any minutes held within the department.
- To communicate with managers and clinical teams to ensure that information requirements are met.
- To be responsible for the preparation and circulation of departmental meeting, information, room bookings and agendas when necessary.
- To assist in ordering of supplies and stock into the department.
- Cover for other members of the admin & clerical team within the service in times of absence
