Due to the relocation out of Manchester of two of our care coordinators, we have two posts come up in our team at once.

The role primarily involves care coordinating a caseload of between 15-24 people who are under our Early Intervention for Psychosis service.

You will be the named person responsible for overseeing the care of those on your caseload, however as we proactively move away from CPA and towards CMHF, those that you work with will be allocated a named team, including staff from our psychology, OT and medic staff groups.

You will work with people on a time limited pathway of 2/3 years depending on their circumstances, and will receive all of the training that you need to enable you to do this to a high level, and feel confident and competent in doing so.

- Managing a realistic caseload of service users who are under the service
- Jointly completing initial assessments with a member of the dedicated assessment team
- Attending and contributing to MDT meetings to discuss and plan care for our service users
- Participating in the duty, back-up duty and weekend working rota
