An exciting opportunity has arisen within Dorset County Hospital for a Discharge Lead. This role has both operational and strategic components and will be key in supporting the delivery of a 'Discharge to Assess' model for patients, while working closely with system partners to deliver integrated care for our patients and population.

The role is suitable for clinicians with a nursing or therapy background, who are passionate about quality and safety of patient care, in particular championing patient independence through robust discharge planning activities and integrated models of care. They will have a strong knowledge of acute and community service provision and be passionate about the role of the clinical MDT in supporting a patient's choice around discharge.

The role will require strong interpersonal skills to develop and maintain strong and effective working relationships across organisational boundaries. As well as effective communication across the multi-disciplinary team, executive teams and commissioners is vital and you will be required to deliver results at pace whilst strengthening quality and safety standards.

**The duties for this post holder will include**:

- To manage the day-to-day operations of the discharge team and lounge, running 7 days a week.
- To lead service development projects associated with hospital discharge and implement the Home First Project.
- To participate in the on-call manager rota.
- Daily senior meetings with health and social partners across Dorset.
- Ensure that complex patients are identified at the point of admission and are proactively supported by the discharge team.
- Review and investigate complaints and incidents to develop a culture of learning both within the hospital and externally.
- To engage patients and families in all aspects of planning, decision making and improvement for a safe discharge.
- To contribute to the identification and delivery of cost saving schemes internally.
