You will be working as part of a large multidisciplinary team that includes Nurses, Physiotherapists, Occupational Therapists, Social Workers, Care Support Workers, Housing Officer, Assessment Officers and Administrators, GPs, Advanced Care Practitioners and Paramedics.

WILL CONSIDER BAND 3’S WITH CONSIDERABLE THERAPY EXPERIENCE AND WILLING TO UNDERTAKE ASSISTANT PRACTITIONER TRAINING AS PER BELOW.

TO UNDERTAKE AN UNDERGRADUATE TRAINING PROGRAMME OVER 2 YEARS. AT THE END OF THE 1ST YEAR THE POSTHOLDER WILL HAVE GAINED ADDITIONAL SKILLS AND KNOWLEDGE EQUIVALENT TO AN NVQ LEVEL 3, PROGRESSING TO YEAR 2 TO ACHIEVE A FOUNDATION DEGREE.

THE POST HOLDERS WILL ALL NEED TO HOLD A CURRENT DRIVING LICENCE AND HAVE THE USE OF THEIR OWN TRANSPORT AS THE ROLE IS WORKING IN THE COMMUNITY.

Due to the nature of the post it is expected that you will have a full driving license with business insurance and will have regular use of an appropriate vehicle (as determined by the Trust).

You will be working along side Physiotherapists, Occupational Therapists and Rehab techs as part of the team using the Single Assessment Process and deliver physical rehab interventions to patients providing rehabilitaion and reablement to adults and older people.

You will have previous experience within a therapy rehab environment ideally including community and/or intermediate care experience.

You will be able to deliver a high standard of evidence based individual care, have excellent interpersonal skills to enable you to work effectively as part of a multi-disciplinary team, and be flexible to meet the needs of a 7 day service.

Working as part of an integrated system with our other Community Services, this is an excellent opportunity to work as part of an innovative team, supporting SCO to deliver operational excellence and deliver a model to support discharge planning at the point of admission (a true Home First Approach).
1. Acute Flow Team - To work alongside patients, families and clinical colleagues during the acute phase of a hospital admission until the point of being ready for discharge. Key requirements will be to communicate with family, obtain all relevant information and prepare processes to ensure rapid discharge working closely with ward teams, identifying required assessments and embedding discharge guidance and strengths-based culture. Included within this team is an admission avoidance team which will pro-actively pull patients from ED & EAU who may be suitable for a community alternative to hospital. The complex discharge team will also form part of this team.
2. Community Home First Hub - This team will receive referrals for all patients with supported discharge needs once discharge criteria is met. The Community HomeFirst Hub will support discharge from hospital same day or the day after where possible and will undertake assessments out-side of hospital setting working with patients, families and clinical colleagues to provide the right care, in the most appropriate place.

You will possess excellent communication skills and have a proven track record of strong organisational skills. You must be able to work on your own initiative to organise and prioritise your own workload and balance this with the demands of this diverse and varied role.

The Service operates a 7 day service, currently covering the hours of 8am to 8pm, including weekends. You will be expected to work shifts to cover these hours and need to be flexible and rotating posts might be an expectation.
