**An exciting opportunity has arisen to become part of our Home First Community Hub Team as an Administrator.**

Salford is currently undergoing service transformation to redesign our Home First discharge processes and pathways. Our vision for Home First is ‘People are in the right place, at the right time - meaning nobody spends more nights away from home than is absolutely necessary’

We have established a Home First Multidisciplinary Community Hub to support people admitted to hospital to be discharged ‘Home First’ and we are looking for Administrators to join the team. You will work with therapy, nursing, pharmacy, social work and voluntary sector colleagues to plan discharge assessments and post discharge support and with the Therapy Team to help plan their daily schedules/letters.

**The Service operates over 7 days, currently covering the hours of 8am to 10pm**

**Main duties of the job**:
The post holder will provide comprehensive and confidential overall office administration including supporting all functions of the Home First Hub. The post holder will play a key role as part of the Hub Team, providing assistance to the Service and Clinical Leads also working collaboratively with other administrative colleagues and support staff.

The role will include developing and supporting the recording of referral and case management by the MDT Hub, attending the daily MDT meetings and supporting management of actions following the MDT meetings.

With your other administrator colleagues, you will provide a service to support the team on a rota basis covering the service 7 days a week during the hours of 08:00 to 22:00 including Bank Holidays.

- Business Administration, RSA 2/NVQ 3 or equivalent in administration, office management or can demonstrate relevant experience

- Ability to deal with sensitive issues.
- Ability to problem solve, plan and prioritise workload
- Ability to work in a high pressure environment
- Understands the importance of confidentiality
- Previous experience of working within a customer care environment
- Knowledge of a range of patient information systems
- IT literate Excellent time management skills
- Adaptable to change and service development
- Knowledge of Microsoft Office software

**Desirable criteria**:

- Knowledge of PAS
- Familiar with use of office equipment such as photocopy/fax, etc.
