- protect and improve landscapes and urban green space
- facilitate the power of local communities to respond to the climate and nature emergency
- build the skills and resilience of young people to be able to realise their potential
- create jobs and training, particularly within the green economy, to support unemployed adults secure employment and develop new skills - with a focus on people with criminal convictions, young adults, care leavers and armed forces veterans

Based in Stockport or Tameside initially, the postholder will work alongside key partners to establish a caseload of people, which could include care experienced young people, economically inactive adults or those aged 50+. Delivery will be a mixture of 121 and group activities and the role will concentrate on promoting independence, regaining and building on social skills, promoting health and well-being, and enabling individuals to achieve positive outcomes through securing voluntary opportunities, educational progression, and paid employment. The post holder will make a real difference to service users lives by developing personal, flexible and realistic support packages that individuals need to turn their life around.

The purpose of the role is to manage a rolling caseload of service users and provide each person with individually tailored support to help them progress into positive activities and destinations as part of their individual recovery journey.

Key responsibilities:
1. Be responsible for the planning, delivery, administration and monitoring of the project activities

2. Identify, engage, and manage a caseload of service users and provide a mixture of intensive 1-2-1 support, group work and training - leading to positives outcomes and destinations.

3. Undertake an initial assessment with each service user to identify their specific needs - leading to the creation of an action plan and regular reviews. Offer ongoing 1-2-1 support and assessment of each service user using agreed monitoring tools.

5. Develop and deliver a range of 1-2-1 and group work programmes, which are specific to the identified individuals needs and contribute to the personal and social development of the service users.

6. Provide ongoing support for each service user after they have moved into a positive progression opportunity with the aim of sustaining this outcome.

7. Be responsible for completion of all administration linked to the project - for example caseload notes, risk assessments and attendance registers.

8. Provide timely and accurate information and evidence to Groundwork and funders as and when required.

9. Work in partnership with other voluntary/statutory agencies and organisations in order to provide the most appropriate support to each service user. Identify progression routes into volunteering, education, and employment.

10. Attend all training and networking events linked to the role.

11. Represent the Trust at relevant internal and external meetings.

12. Undertake any other duties that my reasonably assigned by line manager at Groundwork.

**Salary**: £23,600.00-£25,500.00 per year

- Monday to Friday
- No weekends
