Programme of Care Lead

The programme of care leads are the system level transformational lead for one of the defined care programme portfolios. The ICB will work closely with patients, carers, the public and health and care professionals, and the programme of care lead will be a pivotal role for doing so within their specialist area. Each programme of care lead will need to define their contribution towards these priorities, and would each need to support cross cutting areas, such as personalised care and prevention, digital, workforce and finance.

The role is responsible for driving programmes of work which enable delivery of the integrated care system clinical strategy and forward plan, supporting the high-level ICS priorities, strategy and plans, focused both within their portfolio and across the other portfolios.

**The portfolios are**:
planned care and cancer

mental health, learning disabilities and autism

maternity, childrens’ and young people

urgent and intermediate care (including system coordination centre)

population health management (PHM), health inequalities and long-term conditions (separate job description)

medicines optimisation (separate job description)

Interviews will take place on the 5 th June

The role is responsible for driving programmes of work which enable delivery of the integrated care system clinical strategy and forward plan, supporting the high-level ICS priorities, strategy and plans, focused both within their portfolio and across the other portfolios.

**Each must support delivery of**:
Integrated care strategy - 10 year

Joint forward plan - rolling 5 year

Annual priorities, which for 2023 to 2024 are operational recovery, improving value and transforming care

Portfolio specific national priorities, NICE requirements and local pathways specific areas requiring improvement

Research and innovation opportunities

Each programme of care lead will need to define their contribution towards these priorities, and would each need to support cross cutting areas, such as personalised care and prevention, digital, workforce and finance.

Working directly with clinicians to build consensus on how evidenced best practice is used to transform services, and how and where we provide care to improve health outcomes for our population, the programme of care leads will establish clinical reference groups in partnership with colleagues in the office of the clinical executive, providers, and other system partners.

The programme of care leads will need to convene colleagues from across the system, and secure support from internal ICB teams, notably the transformation support hub, finance, BI, contracting, and support delivery of programmes through the relationship managers and place directors.
