We have an exciting opportunity for an Associate Practitioner post to work as part of the Children and Young People's Continence service. Our Associate Practitioners deliver a high standard of children's continence care particularly in relation to promoting continence and toileting skills for children and young people with additional needs. Providing specialist advice, information and on-going support to children / young people and their families in relation to promoting continence and acquisition of toileting skills; alongside those who receive continence product provision throughout childhood and during transition to adult services.

- To work as part of the Children’s and Young People’s Continence Team (CYPCT), the associate practitioner will be part of providing a service which delivers a high standard of specialist children’s continence care to Children, Young People and their parent/carer's
- The Associate practitioner will aim to support the needs of identified children and their families, particularly in relation to promoting continence, toileting skills and continence product provision assessments - through a variety of development approaches.
- To manage a caseload of children with additional needs providing continence care in a variety of settings; telephone clinics, school based clinics, care in the patients homes.
- To keep accurate, legible, contemporaneous records and written reports as required, with proficient knowledge of information governance.
- Ensure concise and accurate documentation of care interventions, including telephone advice and support are documented and recorded
- Help develop, implement and review clinical protocols and standard operating procedures which assist in delivering safe care and best practice
- Be responsible for decisions made regarding clinical need when assessing individual children who have specific product need
- Participate in the procurement process of the contract for continence products representing children and young people’s needs as being part of the overall continence contract
Work collaboratively with team members, Matrons, Head of CYPCNS, children’s services professionals and other professionals to promote and ensure high standards of professional and clinical practice drawing on national best practice and Trust policy.
The Children and Young People's (CYP) Continence team covers the whole of West Sussex with the addition of Brighton and Hove since early 2018, providing nurse clinics and specialist nurse advice and care for children and young people with Tier 2 continence needs. The CYP Continence team is aligned to the Children and Young People's Community Nursing Services within the Trust.
- To work as part of the Children’s and Young People’s Continence Team (CYPCT), the associate practitioner will be part of providing a service which delivers a high standard of specialist children’s continence care to Children, Young People and their carer's. They will work collaboratively with other professionals who work with children and young people with continence problems. The Associate practitioner will aim to support the needs of identified children and their families, particularly in relation to promoting continence, toileting skills and continence product provision assessments - through a variety of development approaches.
The post holder will also become competent and proficient in their area of practice by undertaking competency assessments specific to their area of clinical practice.
The Associate Practitioner will be both directly and indirectly supervised by a Registered Nurse dependent upon the specific skill being practiced and the level of competency achieved
The post holder will protect, safeguard and promote the well-being of children in accordance with ‘The Children Act’ (1989 & 2004) and ‘Working Together to Safeguard Children’ (HM Gov 2013).- Deliver a high standard of clinical and therapeutic care to a specific caseload of patients within the role and competence of the individual associate practitioner.
