Band 4 Patient Pathway Coordinator (CAT 4 Gastroenterology)

This is a fixed-term post for 12 months - internal applicants welcomed.

We require a passionate, enthusiastic, reliable and hardworking individual to join our team as a Patient Pathway Coordinator to join our Gastroenterology team.

We are looking for an excellent and proactive team leader who can offer commitment in providing support to clinical and non-clinical staff.

The main duties of this role will be within the Two-Week-Wait team to support smooth running of referrals and appointments.

You will also be responsible for FIT testing of our endoscopy waiting list to support the department in prioritisation of demand within the service. You will work closely with the existing 2WW team, the cancer pathway team, Consultants, nurses, registrars and physician associates.

You will be the point of contact for all administrative issues relating to patients' pathway of care, working as part of a multi-disciplinary team and will provide expertise in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly and within required targets.

You will provide a consistent approach across the Trust, using Standard Operating Procedures (SOPs) to ensure that functions of the role are carried out correctly within given timescales.

**The post holder will**:
1/ Be the point of contact for all administrative issues relating to patients’ pathway of care.

2/ Work as part of a multi-disciplinary team and will provide expertise in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly.

3/ Supervise and allocate work to the PPC team.

4/ Ensure high levels of patient and clinician satisfaction by being an accessible, customer-focused and knowledgeable point of contact.

5/ To provide a consistent approach across the Trust, using Standard Operating Procedures (SOPs) to ensure that functions of the role are carried out correctly within given timescales.
