**Community Support Worker Physical And Sensory Impairment West**

Permanent, Full Time

£23,456 to £29,933 per annum

The Physical and Sensory Impairment Team West supports adults over 18 years of age who have long term complex physical and sensory impairments. We cover the West of Essex from Uttlesford to Waltham Abbey. We are also on the border of LB of Redbridge, Hertfordshire and Cambridge.

In our team share you will work as part of a multi-disciplinary team. We aim to support adults, and their families, who have complex social care and support needs and are more vulnerable as a result of disability, exclusion, or diminished capacity. We aim to safeguard and protect the adults that we work with balancing independence, control and being able to live as independent and a fulfilling life as possible.

As a Community Support Worker, your role will include undertaking Assessments and Reviews in line with our Statutory duties under the Care Act to identify eligible needs and to then generate a person-centred and outcome focused support plan. An essential attribute for the role is to be ‘curious’ as thinking ‘outside the box’ is a must to achieve the best outcomes and to fully engage and utilise all that our community has to offer. Other roles include supporting with the teams duty function, supporting as appropriate with safeguarding intervention and multi-agency working.

**About You**

You’ll bring a strong ability to build effective relationships with adults and their families in times of challenge and crisis. You will have the drive and passion to want to make a difference, and a willingness to learn. Ideally you will have some experience of activity-based work with adults and some knowledge of legislative and policy guidance applicable within social care settings.

**Please note that the job role requires you to be mobile throughout an operational area. Therefore, the post holder will need to have a driving licence and access to a vehicle, **or the ability to meet the mobility requirements of the role through other means.**
