Deliver a range of evidence based brief interventions on a range of health, lifestyle and behaviour related issues under the direction of the EMHP and/or Supervising Practitioner. This may include Providing brief advice Co-deliver group interventions Guided Self help Signposting Creating and distributing resources Assisting with delivery of WSA activities Supporting the development and delivery of our digital offer Participate in and contribute to group team business/clinical meetings to keep up to date with organisation information and the production of activity reports. Ensure all communications are received and dealt with in a courteous, sensitive and professional manner. Liaise with other agencies as necessary to ensure appropriate resources are available to each patient.

Ensure all changes in the need of the patients are effectively reported to the designated qualified practitioners as soon as is practicable and at least within 24 hours.
