The Community Neurological Rehabilitation Team (CNRT) inChichesteris expanding and developing stroke services. As part of this we have an exciting new opportunity for a Caseload Co-ordinator. CNRT Chichester is a busy multidisciplinary team providing specialist neurological rehabilitation to individuals in the Western side of West Sussex. We receive referrals from multiple agencies, for example NHS and social care providers, consultants, specialist practitioners and some self referrals.

We are looking from someone who is passionate about providing exemplary personcentredcustomer care to those on our neurological caseload. Being the point of contact for the service you will also need excellent communication, organisational and prioritisation skills. This is a rewarding role and you will be fully supported by our friendly and knowledgeable expanding team of registered and unregistered health professionals.

To be the point of contact for CNRT Chichester for referrers, professionals and those on the team caseload, dealing proactively with enquiries and escalating any concerns where necessary.

To accurately process and coordinate all referrals received by the team within the specified time frames to ensure a timely response can be actioned by the team.

To be proactive in ensuring all of the relevant information is available to the professionals to assist them with their decision making and prioritisation.

Where appropriate, to undertake telephone triage reviews for new referrals and long term clients to gather information and help determine the appropriate professional and intervention required.

To provide a person centred approach to ensure all clients’ and carers’ views and opinions are considered and all people who make an enquiry are given respect, dignity and understanding.

To help to plan, organise and co-ordinate the daily stroke allocation meeting ensuring that all relevant information is available to team members.

To undertake surveys or audits as required by the team and to be one of the named SSNAP(National Stroke Survey) audit links
